• Up to 1115K salary (DOE), Hire Bonus, Annual Bonus, Mileage Reimbursement
• HIGHER SALARY IS POSSIBLE DEPENDING ON WEEKLY VISITS / TERRITORY

• Reasonable patient load and no call
RN Field Case Manager for skilled Home Care. Available territories include Bronx or Manhattan or Westchester
What you will be doing:
- Field RN Case Manager providing home health visits for skilled nursing care
- Admissions, OASIS, create plan of care, care coordination, re-certifications, discharges
- Provide supervision, skilled nursing care
- Ensure documentation is completed
Skills you need:
- Home Health experience highly preferred
- NY state RN license
