Case Manager RN Job Description
Calling all quality-minded, impact-driven Home Care Nurses! EverCare is currently seeking several Case Manager, RNs to cover Orange, Dutchess, and/or Rockland service areas. As a Case Manager you will use your specialized nursing skills to provide critical nursing care to achieve the best possible physical health and quality outcomes for our patients in the comfort of their own homes. Using appropriate preventive and rehabilitative nursing procedures, you will interact with patients and their families in support of their health and treatment goals. We currently offer full-time, part-time, per visit, and weekend opportunities to meet the needs of your busy life.

Minimum Qualifications:
Bachelor’s degree in nursing with current NYS registration; or
RN with a bachelor’s degree in health | human services; or
RN with two years’ experience in HOME CARE
Valid NYS Driver’s license and reliable transportation
