$5000 Sign-on Bonus

Job Description: Serving adults with traumatic brain injury and other physical and/or neurological impairments. This Case management position includes case management, assessment, training, and supervision of, or assistance to, an individual with issues related to activities of daily living and/or other instrumental daily living skills such as meal preparation, budgeting, and medication management and/or support in our day program depending on need. Experience with case management and implementation and management of behavioral treatment plans is a plus.

Experience: Dependent on current Department of Health Qualifications

Experience in:

• assessments
• case management
• writing service/treatment plans
• teaching individuals with disabilities, including traumatic brain injury
• implementation and management of behavioral treatment plans

Education: Dependent on current Department of Health Qualifications

• Minimum requirement is an Associates Degree + 5 years experience
