Job description

Job Summary The Registered Nurse Case Coordinator identifies and implements nursing interventions and evaluates results for a wide variety of patient populations, directs and acts as a resource to other members of the nursing team as assigned, and promotes compliance and support for policies.
The primary responsibilities for this position consist of providing total, comprehensive, continuous, and patient-centered care which includes assessing, documenting, planning, implementing, teaching, developing, and evaluating a comprehensive nursing plan of care; delivering direct patient care; ensuring patient satisfaction; training and delegating duties; and adhering to University policies and procedures as well as federal, state, and local regulations.

Minimum Qualifications

  • Valid California Registered Nurse License without restriction.
  • Graduation from an accredited school of nursing.
  • Minimum of two (2) years of recent RN experience in ambulatory clinic.
  • Two (2) years of RN experience in acute care, ICU, or Emergency Department.
  • Recent Telephonic triage experience.
  • American Heart Association (AHA) BLS CPR Certification or UCDH-issued CPR certification.
  • Ability to read, write, speak, and understand the English language.
  • Ability to utilize medical terminology appropriately.
  • Knowledge of current trends in nursing and telehealth.
  • Commitment to excellence in patient care.
  • Effective interpersonal skills.
  • Demonstrates accountability and professionalism.
  • Adapts to change in a positive, professional, and productive manner.
  • Knowledge of legal implications in clinical practice and documentation.
  • Knowledge of performance improvement and evidence-based practice.
  • Ability to assess, plan, implement and evaluate patient care, taking into consideration protective interventions, development (behavioral and physical), and physiological characteristics specific to the patients.
  • Advanced clinical knowledge in a variety of modalities, or significant expertise in a specialty area.
  • Advanced ability to assess patient needs and make appropriate referrals for specialized support services and programs.
  • Strong proficiency in using computers, software, and web-based applications (e.g., Windows-based software including Microsoft Word, Excel, Outlook, clinical-based computer systems and software).
  • Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
  • Excellent conflict resolution skills.

Preferred Qualifications
  • CCM or other appropriate case management certification.
  • BSN preferred.

Department Description
The Care at Home, Connected Care Center is dedicated to providing comprehensive and innovative healthcare services that bridge the gap between traditional medical care and modern technology. Our department focuses on improving the health of our patients with digitally-enabled care with service such as remote patient monitoring, on-demand urgent care, and mobile medicine service lines. By leveraging advanced digital tools and a team of qualified healthcare professionals, we aim to improve access, convenience, and quality of care for our patients.
Position Information
  • Salary or Pay Range: $70.21 - $94.47
  • Salary Frequency: Hourly

Physical Demands and Work Environment
  • Has mandatory reporting responsibilities for domestic violence, child abuse and elder/dependent abuse.
  • Works overtime as defined by the collective bargaining agreements, floats to other areas and works weekends and holidays.

  • This position is a critical position and subject to a background check and drug screen. Employment is contingent upon successful completion of background investigation including criminal history, identity checks and drug screen.
