REGIONAL HOME HEALTH ANDHOSPICE MEDICAL SOCIAL WORKER/BEREAVEMENT COUNSELOR

Part Time - Days

Covering Forsyth, Davie, and Davidson Counties

Job Summary:

• Patient Care Services for Home Care and Hospice
• Anticipatory Grief and Post Grief Support
• Access community resources
• Exhibit professional verbal and written communication and documentation
• Professional education

Job Requirements:

• Baccalaureate of Social Work degree (BSW) or Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education.
• Requires strong interpersonal skills, with the ability to effectively communicate within a multidisciplinary setting.
• Employee must have own reliable transportation.
• Requires knowledge of computer skills and the ability to use company approved documentation program.
• Requires ability to work autonomously with skills in organization, timeliness and ability to provide effective care for hospice patients and their family unit.
• Requires knowledge that allows for identification and reassessment of social, emotional, financial and environmental factors which may affect the medical plan of treatment and desired outcome.
• Knowledge of the stages and reactions to anticipatory grief and grief work.
• Responsible as a member of the interdisciplinary team to provide patient centered supportive services to help meet the patient/family social, emotional and psychological needs.
