Job Responsibilities

ISP Implementation

∙ Complete Assessments Required by the County Board

∙ Attend ISP Meetings

∙ Monitor Goals and Documentation

∙ Respond to UIR’s

∙ Shepherd Health and Living, LLC. Liaison to Guardian and County Board

• Timely and Effective Follow Through to Enhance Professional Services
• Complete Doc Sheets for All Individuals on Caseload – Review Completed Doc Sheets
• Office and Residential Files Completed (Files Must All Be Similar Agency Wide)
• Complete IST’s for Each Client – One Copy to Client File, and One Copy to Employee File
• Complete MARS Ensuring Dr’s Orders are in Place
• Assuring Client Cash on Hand is Logged in the Homes and Money is accounted for

∙ Hiring; Interviewing, New Employee Requirements

∙ Train New Staff on ISP

• Supervise Direct Care Staff

∙ Evaluations and Discipline

• Daily to Weekly Staff Schedules
• In Homes at Least Once Per Week Checking Documentation and Fixing Issues Immediately (Medications, Groceries, Cleanliness, etc). Please Keep a Log of Dates and Times In Homes (Will Be Reviewed By DOO/Operator At Any Given Time)

Services

• Coordinate Residences
• Pharmacy Services
• Routine Medical Services, Schedule, Appointment Assistance, Train Staff
• Report Accurate Staffing Hours Weekly
• Ensure/Oversee All Client Meds are Checked In
• Follow DD Rules and Regulations

Emergency Services

∙ Rotate On-Call for Problem Solving, Staff Call Offs

∙ Coordinate Emergency Placement Services

Other Non-Specified Duties

• Keep Track of Comp Hours
• Turn in Timesheets with Vacation and Sick Days Usage and Milage for reimbursement to Payroll Coordinator

Job Type: Full-time

Pay: From $70,000.00 per year

Schedule:

• 8 hour shift
• Day shift
• Holidays
• Monday to Friday
• On call
• Weekends as needed
