Summary

  • The Refugee Nurse Navigator will work closely with refugee physicians to coordinate refugee care plans.
  • Interface with multi-disciplinary team to maintain and implement up-to-date, coordinated care plans and keep open lines of communication with all healthcare team members on behalf of the refugees. Follow Individuals with special needs until issues have been addressed by a healthcare provider.
  • Oversee patient referrals and clinical protocols, act as liaison with the medical community, and provide health education for refugees and their families.
  • Monitor interpreter translation services received in the field and in clinics to ensure effective communication between medical staff and patients.
  • Assist children with receiving proper vaccinations to meet school enrollment requirements.

Essential Duties and Responsibilities
  1. Assists supervisor with employee interviews, act as lead for preceptor team, incorporates diversity training, identifies areas of concern to address with staff and refugees, develops care plans with timelines, and facilitates development of evaluation tools for quality assurance reviews. Identifies areas that could provide needed services, helps with setting up of appointments, and ensures comfort of refugee when riding the bus or alternate type of transportation to needed appointments.
  2. Performs physical and needs assessments and gathers history and pharmaceutical history.
  3. Collects specimens and carries out all other physician orders.
  4. Takes documentation by exception, especially for the initial visit.
  5. Oversees case management of refugees. Follows each individual to ensure medical appointment attendance as required.
  6. Offers education and counseling to refugee families along with refugees.
  7. Assesses country of origin and distribution of parasite medication.
  8. Performs quality assurance activities.
  9. Prepares statistical reports sent to the Texas Department of State Health Services.
  10. Performs all other duties as assigned.

Minimum Requirements

NOTE: YOU MUST FILL OUT THE WORK HISTORY and EDUCATION SECTIONS OF APPLICATION TO SHOW YOU HAVE YEARS OF EXPERIENCE/EDUCATION AS REQUIRED BY HIRING DEPARTMENT OR BE DISQUALIFIED.

TO APPLY, must have a Bachelor of Science degree in Nursing (BSN) or higher + Two (2) full-time years of public health and/or supervisory experience, having been responsible for the work output and performance evaluation of subordinates.

OR

TO APPLY, must have an Associate's degree in Nursing (ADN) + Four (4) full-time years of public health and/or supervisory experience, having been responsible for the work output and performance evaluation of subordinates.

TO APPLY, must have a Current Texas or multistate compact (including Texas) RN license.

TO APPLY, must have Texas RN license required within ninety (90) days of hire.

Ability to start and manage intravenous therapy, assist physician with total physical assessments, and run EKG machine and other equipment as ordered by physician. NOTE: MUST PROVIDE LICENSING AGENCY AND LICENSE NUMBER IN "CERTIFICATES AND LICENSING" SECTION OF THE APPLICATION OR RISK DISQUALIFICATION.

Ability to start and manage intravenous therapy, assist physician with total physical assessments, and run EKG machine and other equipment as ordered by physician.

If hired, must provide proof of educational attainment
at New Hire Processing or during the promotional process.

Tarrant County will conduct background checks on new hires that will include a criminal background check related to convictions and deferred adjudications in the past Seven years and may include credit reports, motor vehicle records, employment records and educational attainment. Physical Demands and Work Environment & Other Requirements

While performing the duties of this position, the incumbent is regularly required to bend, carry, climb stairs, grasp/squeeze, use hand tools, hear, tolerate hot and cold inside temperatures, keyboard, kneel, lift below waist, lift between waist and shoulder, lift overhead, lift to the side and twist, use material handling equipment, tolerate noise, tolerate hot and cold outside temperatures, pull, push, reach, reach overhead, perform repetitive tasks, see color, see far and near, sit, squat, stand, stoop, talk, twist, use his/her hands, drive, walk, and work alone and with others. There is exposure to active infectious tuberculosis, patients that are HIV positive and/or Hepatitis B and C positive, and patients that are infected with parasites. Bloodborne pathogens protocols must be carefully followed.

Regular attendance is essential. Incumbent must arrive at work on time, prepared to perform assigned duties and work assigned schedule. Incumbent must have the ability to work well with others. Incumbent must be able to perform the essential functions of the position without posing a direct threat to the health and safety of themselves and others.
