Description:
This will be a full-time position, M-F, 40 hours/week. The job tasks and description will be the same as other Qi/PHMs in the Quality and Population Health Department and include developing Quality Improvement strategies, managing improvement projects, and supporting teams to monitor and sustain improvements. The position would be responsible for supporting the scope of work related to the MassHealth requirements for participation and performance in the Health Equity Incentive arrangement.

  • The Quality Improvement & Population Health Manager is responsible for the development and management of value based care driven initiatives for Quality Improvement. Partnering closely with Medical Director of Quality, Quality Improvement Champions and Population Health Staff you will guide multidisciplinary teams through various Quality Improvement projects. You will play a key role in ensuring that ENBHC performs at the highest levels on standardized, national measures of clinical quality care. The Quality Improvement & Population Health Manager ensures the accurate and timely collection of data from clinical records, identifies opportunities for improvement, communicates and works collaboratively with multidisciplinary improvement teams and facilitates the creation and implementation of improvement in care process.

Duties & Responsibilities:
  • Develops and updates quality strategies in conjunction with Quality Medical Director, including use of other internal resources for organizational priorities, such as NCQA/PCMH, ACO, HRSA, UDS and Meaningful Use/ QIPS.
  • Plans, organizes, and prioritizes work to meet organizational goals, objectives, and deadlines.
  • Oversee Quality Improvement efforts across East Boston and South End sites according to organizational priorities.
  • Oral & or Power Point presentations of QI project design and outcomes to internal audiences at department and committee meetings.
  • Independently leads, facilitates, and manages multidisciplinary improvement projects.
  • Build capacity of leaders across sites to lead Quality Improvement initiatives and Plan/Do/Study/Act (PDSA) cycles.
  • Promote a culture of Quality Improvement (QI) across the organization.
  • Support regulatory compliance and submission of national, state, and local certifications, audits and surveys related to quality. (i.e. ACO, PCMH, UDS, HRSA, and other Certification Programs).

Other Duties & Responsibilities:
  • Reads, understands and complies with all Standards of Conduct, JCAHO policies, CMS Pace Program Regulations and Standards, applicable State regulations and guidelines, ESP policies and procedures and all laws related to position responsibilities and employment at EBNHC.
  • Upholds procedures and systems to safeguard the confidentiality of all patient and employee information.
  • Upholds safety policies, practices, and procedures including safety, fire safety, electrical safety, proper body mechanics and material handling, office/ergonomic safety and other employee safety measures.

Other Duties & Responsibilities Organizational Competencies:
  • Education:
  • MS in Health Profession or related public health field or BA degree accompanied by at least 3 years of project management or continuous Quality Improvement experience.

  • Experience:
  • 3+ years LEAN/Six Sigma, Model for Improvement or similar experience in process Improvement and project management. Significant related experience in data management.

  • Skills/Abilities:
  • Technology proficiency required; Proficiency in EPIC, Expertise in Word, Excel, Visio/ Flowcharting & Mapping and PowerPoint.
  • Expertise in EPIC and RWB.
  • Knowledge in data management, data management systems, and spreadsheet manipulation helpful.
  • Ability to perform multiple activities efficiently in a timely and accurate manner.
  • Excellent attention to detail.
  • Excellent organizational skills
  • Excellent written and oral presentation skills, with the ability to engage, inspire, build credibility and trust.
  • Mastery of principles of process Improvement and project management and ability to apply those principles in a broad range of projects and initiatives.

Salary: $59,537 - $68,467
