Job Description
This position supports the Quality Improvement Program at MonmouthCares through effective and efficient data collection and analyses as well as timely reporting to various departments in support of Agency's management activities.
Areas of Responsibility
- Assist with maintaining and managing the SharePoint database platforms for accurate and consistent data collection.
- Track data monthly and complete annual, quarterly, and monthly reports in the following areas, among others:
- Agency Incidents
- Unusual incidents
- Technology incidents
- After Hours calls
- Hospital screenings and admissions
- CM enrollments/transitions
- Re-enrolled youth
- Facilitate the Wraparound Measurement program, collecting data, designing campaigns, working with outside vendors, modeling and analyzing data, and reporting Wraparound fidelity data.
- Support the agency Medicaid compliance program by completing monthly and annual PERM reviews, conducting monthly quality calls with caregivers, and reviewing care management documentation.
- Support the agency stakeholder experience program by sending and tracking monthly satisfaction surveys, annual post-transition surveys, and an annual community stakeholder survey.
- Participate in and provide data analysis support for all Quality Team special projects.
- Contribute to the support function of the Quality-Technology-Compliance department.

- HIPAA-HITECH
- Working knowledge of voice, video, Internet, and mobile connectivity
