Prime Home Health is seeking an experienced LVN as full-time Quality Assurance Coordinator for our San Antonio office.
You will be overseeing all aspects of the Quality Improvement Program, monitoring, analyzing, trending and utilizing data for the improvement of the Agency and the services provided. A minimum of 2 years experience in QA for a large Home Health agency is mandatory.
Responsibilities include:
- Conducting ongoing, comprehensive, integrated agency assessment of the quality and appropriateness of the care provided, including services provided under arrangement
- Aggregate and trend data collected to correct identified problems in collaboration with management
- Monitor all service providers for effective interchange and coordination of care in providing
- Prepare annual quality improvement report as part of annual agency
- Demonstrates an in-depth knowledge of, ensures compliance with, all local, state, and federal laws relating to all of the operations of the agency.
- Assists with direction of programs through delegation of responsibility to administrative and supervisory
- Assists with establishment of a public relations program for interpretation of the agency's services and to foster good working relations with physicians and community agencies.
- Carries out other duties as assigned by the DON
Requirements:
- Licensed LVN in State of Texas
- ICD-10 Coding Certification a plus
- Experience in Hospice, Home Health and in quality improvement
