Primary Purpose of Position: Canyon Hills Treatment Facility is a Psychiatric Residential Treatment Facility (PRTF) that is committed to providing quality Psychiatric Residential Treatment services in accordance with the Local Management Entity (LME) and North Carolina Division of Developmental Disabilities, Mental Health and Substance Abuse Services (DD/MH/SAS) and rules and regulations regarding residential treatment facilities for children and/or adolescents up to 21 years of age. Canyon Hills offers psycho educational and relational support, behavioral modeling of interventions and supervision to the consumers residing in Canyon Hills. These preplanned therapeutically structured inter mentions occur as required and outlined in the consumer's treatment plan.
POSITION SUMMARY:
- QP will provide quality case management and professional supports and services to consumers as assigned by the Immediate Supervisor.
- QP will work with local agencies, service providers, and other healthcare agencies to facilitate sending and receiving collateral information, treatment referrals. and ongoing communications to support patient's successful in-patient treatment and discharge.
SKILLS:
- Knowledge of developmental psychology and heeds of various populations
- Knowledge of short term/stabilization mental health treatment
- Professional verbal and written skills needed for communication the consumer status and treatment need to payers, families, professionals, paraprofessionals employees of the hospital, and members of the consumer's community.
- Ability to maintain professional and confidential records according to the payor source
- Ability to always maintain patient confidentiality and high ethical standards
- Ability to maintain professional relationships with patients, patient's family members, staff and community personnel and documentation of such contacts
- Knowledge of appropriate assessments. diagnoses, and interventions
- Knowledge of in lieu management
DUTIES:
- Ensure CFT meetings are held timely, and PCPs are completed and submitted for authorizations at least 14 days before Service authorizations end.
- Submit weekly behavioral reports on consumers to guardians, MCOs and team members.
- Maintain a professional environment in the Clinical office
- Ensure family visitations are scheduled according to work schedule of staff
- Responsible for assuring adherence to all policies and procedures of the facility as well as to the state requirements.
- Ensure the facility maintains a safe and secure environment for the residents and the staff
- Coordinate, Report, Document, and Schedule CFT Meetings for assigned Side
- Review and Chart all Direct Care notes for assigned Side
- Ensure the current documentation, notes, and misc. information are filed in the consumer record
- Complete and Submit Monthly Reauthorization with the SAR/ TAR, with direction from Clinical Director, etc...
- Complete NC TOPPS timely
- Attend and participate in scheduled administrative staff meetings as directed by the Immediate Supervisor
- Serve on and co-facilitate internal agency, committees, such as tasks forces, Quality Assurance/ Quality Improvement (QA/Q1), and other committees.
- Provide special consultation in consumer and stakeholder satisfaction studies and other qualit improvement activities.
- Report to facility according to schedule agreed upon by the administrative team
- Attend weekly supervision meeting, face to face, with immediate supervisor
HOURS OF WORK: Varies according to the needs of the facility
NOTE.Some weekend and evening work may be required by approval only. This position is exempt under federal labor laws and regulations.
MINIMUM EDUCATION, TRAINING ANDEXPERIENCE REQUIRED:
Must meet the following criteria for "Qualified Professional” within the mh/dd/sas system of care:
(a) an individual who holds a license, provisional license, certificate. registration or perm it issued by the governing board regulating a human service profession. except a registered nurse who is licensed to practice in the State of North Carolina by the North Carolina Board of Nursing who also has four years of full-time accumulated experience in mh/dd/sa with the population served.
(b) a graduate of a college or university with a master’s degree in a human service field and has one year of full-time, post-graduate degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional who has one year of full-time, post-graduate degree accumulated supervised experience in alcoholism and drug abuse counseling.
(c) a graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time. post-bachelor's degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional who has two years of full-time, post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling:
(d) a graduate of a college or university with a bachelor's degree in a field other than human services and has four years of full-time, post-bachelor's degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional who has four years of fulI-time, post- bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling.
