Job Posting: RN Case Manager for CAP C Program (Community Alternatives Program)
Hybrid Remote and In person work required.
Work will be conducted as a combination of work from home, and in person home visits within our service area (The following counties of NC: Pender, Duplin, Sampson, New Hanover, Brunswick, Columbus, Bladen and Onslow). Training and team meetings will also be conducted in person.

The Community Alternatives Program (CAP-C) is a North Carolina Medicaid waiver program that provides home and community-based services to medically fragile children as an alternative to institutionalization or facility placement. Through the CAP-C Program we are able to assist medically fragile children and their families who desire to receive services and supports in their home and community environments. These services allow the beneficiary to remain in their home and assists in supplementing their natural supports with formal supports to allow for the maintenance of their health, safety and well-being.
Qualifications:
- Registered nurse who holds a current North Carolina license
- Two-year or four-year degree
- One year case management experience in homecare, long-term care, personal care or related work experience
- Valid North Carolina drivers' license
- Spanish speaking preferred
Job Description:
As a CAP CCase Manager, you will be responsible for assessment, care planning, monitoring and facilitation of services for our beneficiaries. We work as the link between families and the Division of Health Benefits of NC for service requests and approvals, as well as the link between families and local service providers for the facilitation of their approved waiver services.
Specific Roles and Responsibilities:
- Assessing the needs of medically fragile children and their families
- Monitoring of health, safety and well-being in the home and community
- Creating and revising care plans for CAP C waiver and non-waiver services
- Collaborating with local providers for facilitation of in-home services
- Linking to local community resources
- Working within part of the RN/SW interdisciplinary team
Must excel in:
- Effective and prompt communication, both written and oral
- Up to date computer skills including video conferencing
- Creative problem solving
