Provides care coordination, care plan development, and skills assessments (you will collaborate with a RN on the team to ensure the patient's medical needs are met ).
Anticipate and assist with symptom management.
Participate in family support, teaching, discharge planning and continuity of care across sites of care, in collaboration with the primary care team.
Participate in both formal and informal education for all disciplines.
Job Responsibilities:
Collect information from all disciplines and data sources, including the family, to synthesize a holistic care plan based on the needs of the child and family.
Anticipate child needs. Work with patients and families to determine their understanding of the child’s condition, its treatment, and their treatment options. Monitor the client’s progress, provide for continuity in interventions over time and through systems, and modify care plans as needed. Participate in health care planning and decision making. Act as a resource person for clinical information and education as requested by community agencies. Assess the environment for potential need for consultation.
Plan educational interventions which are consistent with individual and/or organizational needs and goals, and evaluate change/outcomes. Implement patient and family education in collaboration with the interdisciplinary team.
Contributes to team effort by accomplishing related tasks as needed.
Qualifications and Education Requirements
Bachelor's degree in social work from an accredited school of social work, and one year of directly related community experience
Bachelor’s degree in a human services or equivalent field from an accredited college or university with two (2) or more years of community experience
All will complete DHB certified training program within 90 calendar-days of employment.
Preferred Skills
CAP/C Case Management Experience, bilingual (Spanish).
