The City of Long Beach, Department of Health and Human Services is recruiting for a full-time, unclassified, at-will Public Health Professional II to serve as the HIV Care and Prevention Outreach Coordinator in the Community Health Bureau, Clinical Services Division. Reporting to a Community Program Specialist V, this position will provide oversight and coordination of HIV outreach/linkage to care services, prevention PrEP and PEP program, and Ending the HIV Epidemic DIS services; and conducts client recruitment, screening and assessment of HIV and sexually transmitted infections (STIs), substance abuse risks, medical and psychosocial service needs as a member of a multidisciplinary treatment team. This position is located at the main health department office located at 2525 Grand Ave, Long Beach CA 90815.
- Monitors and maintains program compliance including data oversight, service delivery, and prepares monthly and annual reports;
- Supervises, trains, and monitors program staff;
- Performs HIV testing, counseling, and referrals;
- Oversees collaboration with agency HIV/STI surveillance team and uses surveillance data to locate and engage HIV newly diagnosed or lost to care individuals, utilizing CalREDIE;
- Maintains coordination with community-based organizations (CBOs) and provides support during various outreach events in the community;
- Develops materials for and provides presentations related to HIV, STIs, and PrEP/PEP; organizes and participates in agency and community meetings; and participates in local planning groups;
- Provides client services supporting linkages to care, retention in care, and re-engagement into care;
- Provides risk-reduction, treatment advocacy and adherence support, health education, and partner services to individual clients as well as in a group setting;
- May assist in the development and implementation of grant applications; and,
- Performs other related duties as assigned.
- Bachelor's Degree in Public Health, Public Administration, or a closely related field (*proof required).
- Program management experience in a public agency beyond the minimum requirements may be substituted for education on a year-for-year basis.
- One (1) year of professional experience coordinating and/or supporting community-based social service or PUBLIC HEALTH programs.
- Valid motor vehicle operator license.
- Required to obtain a California motor vehicle operator license within 6 months of employment if license is out of state. DMV K4 driver license information report printout required during onboarding if selected for position.
- Master's Degree in Public Health, Social Work, Public Administration, or a closely related field.
- Ability to work evenings, nights, weekends, holidays, and on-call during emergencies as needed.
- Experience with various data systems (ARIES, EHR, CalREDIE and/or others)
- Experience designing, conducting and evaluating community engagement campaigns
- Experience completing communicable disease investigations
- Experience facilitating and/or leading group meetings
- Experience facilitating and linking referred clients to sexual health services
- Bilingual skills (Khmer, Spanish, Tagalog, and/or Vietnamese)
- Demonstrates skill in patient assessment, intervention, planning, and evaluation.
- Demonstrates initiative, good judgment, and the ability to make decisions independently.
- Demonstrates skill in working with high-risk and mentally ill populations.
- Strong communication skills, verbal, non-verbal, written and oral.
