Program Specialist, Home Life Services - Full Time, Jamestown, ND
JOB SUMMARY
The Program Specialist is responsible for performing tasks related to the delivery of comprehensive Person Centered Service Plans/Individual Service Plans that include health, medication and personal cares, therapeutic, vocational and/or recreational activities for the individuals being served by the Anne Carlsen Center in the Home Life Services program. This position will carry specific responsibility in the guidance, training, coaching, and mentoring of the Home Supervisors and Direct Support Professional staff in implementing service plans and cares. This position will work closely with the individual program coordinators, RN Care Coordinators, Behavior Analyst, and the education, nursing, and therapy service personnel. The Program Specialist will develop and promote positive relationships with the client, their families, and external agencies.
RESPONSIBILITIES:
This position assumes the authority, responsibility, accountability and gives explicit direction for the development, implementation, organization, ongoing monitoring, evaluation, and coordination of Home Life Services programs and services, i.e., PCSP's/ISPs and behavioral support services, based on information gathered from the individual, families, medical personnel, internal team members and pertinent outside agency personnel. This position is responsible to help maintain a cost effective budget especially regarding the supplies and equipment needed for client program plan implementation. This position serves as a positive role model for staff.
This position works under the direction of the Operations Manager and resolves unusual or serious issues with the Operations Manager.
Education and/or years' experience required:
  • A bachelors degree in social work, behavioral science, education, or related field. Experience in developing, implementing, monitoring, and modifying individual service plans/IEPs/Therapeutic plans preferred.
QUALIFICATIONS
(minimum requirements)
  • Must have the ability to demonstrate effective skills in organization, supervision, management, leadership, and communication- both oral and written. Must be able to demonstrate effective interpersonal skills; being tactful, courteous, and diplomatic with internal and external contacts. Ability to adapt to rapid change, and be able to intervene promptly in crisis/emergency situations.
  • The incumbent must have exceptional listening skills and intrapersonal skills with the ability to gather information from clients regarding services needed. The incumbent must be able to demonstrate the ability to look at a current program and/or home, school, or work situations and synthesize formal and informal information into a workable program plan and develop the contacts and knowledge to coordinate resources.
  • The incumbent must be able to support, interpret, implement, and communicate Anne Carlsen, State, and Federal policies, procedures, laws, rules and regulations for the department.
  • Understand and have a working knowledge of Windows, Intranet and Internet technology. The incumbent must have the ability to access, manipulate, organize, and relay information in meaningful ways using: email, etc.
