Under the supervision of the Clinic RN Manager, manages flow of clinic leading all staff in provision of medical and nursing care, ensuring high quality standards.  Coordinates services with primary care Providers; acts as primary liaison.  Triages and assesses participants as needed. Provides skilled nursing care to participants, implements orders, develops appropriate plans of care, provides participant, caregiver, family, and staff teaching and communicates participant's condition to caregivers, primary care, and the interdisciplinary team.  In the absence of the Clinic RN Manager, assumes his/her responsibilities, including oversight of all responsibilities for LPN, Clinical Assistants, and Clinic Clerk.  May need to provide position support at another Inspira LIFE Center due to operational need.

As a multisite provider of PACE community-based services for seniors in Cumberland, Gloucester, and Salem Counties, an Inspira LIFE employee may work at either or both the Vineland or Williamstown LIFE Program as census changes or operational needs occur to meet the needs of our participants.

Education & Experience:

  • Graduate of a school of professional nursing, BSN required.
  • Minimum of 1 year working with the frail or elderly.
  • 2 years of Nursing Experience.

Certification/Licensure:

  • RN License - New Jersey.
  • NJ State Choice Assessment Certification to complete level of care assessments-- This training will be provided by Inspira LIFE within the first 12 months of employment. 
  • BCLS required.
  • Valid Driver's license, proof of insurance and have a reliable means of transportation as described in Fleet Policy # TR 118.  

Position Responsibilities

  • Conducts an in person comprehensive initial nursing assessment with new LIFE enrollees.  Coordinates with the interdisciplinary team to develop a comprehensive plan of care for each participant.  Provides ongoing assessment, monitor health problems and health status as needed, and implements nursing care plans. 
  • Conducts an in-person nursing reassessment annually, semiannually, and as needed.
  • Develops a plan to meet skilled, intermediate, and personal care needs, and sets long and short-term goals.
  • Functions as a member of the interdisciplinary team.  Maintains regular attendance and participates in the interdisciplinary team meetings; communicates participant changes, collaborates on care planning decisions and coordination for 24-hour care. Effectively communicates in interdisciplinary team meetings, family meetings, and clinic meetings.
  • In Clinic RN Manger's absence, supervises LPN, Clinic Assistants and Clinic Clerk. 
  • Evaluates overall effectiveness of the clinic.
  • Assists in the treatment, examination, and testing of participants.
  • Observes, records and reports participant's condition and reaction to drugs and treatments to providers.
  • Working with Clinic Practice Coordinator, oversees appointment bookings and ensures preferences are given to participants in emergency situations.  Maintains timely flow of participants.
  • Responds to and performs triage of incoming telephone calls.
  • Implements the orders written by Primary Care Providers timely.
  • Notify the Primary Care Provider of any marked change in the participant's condition.
  • Provision of emergency care including arrangements for transportation.
  • Instructs and educates participants and family regarding medications and treatment instructions.
  • Maintains and reviews participant records, charts, and other pertinent information. 
  • Acts only within the scope of his or her authority to practice.
  • Ensures that exam rooms are stocked for necessary supplies and materials.  Ensures cleanliness.  Ensures that appropriate staff maintains a list of medical supplies.
  • Provides on-call coverage on a rotating basis with other staff to troubleshoot, advise and coordinate participant care.
  • Works weekends on a rotating basis to cover homecare needs of participants.
  • Demonstrates positive interpersonal relations in dealing with fellow employees, supervisors, and other department employees to maximize positive employee relations and productivity.
  • Maintains a clean, safe, clinic environment, promotes accident prevention, and reports potential hazards.
  • Demonstrates professional practice in patient care by use of established standards in planning, organizing, implementing, and evaluating.
  • Maintains current, written records in compliance with regulatory standards; prepare a variety of written reports and correspondence.
  • Makes home visits and nursing facility visits as needed.
  • Works with Homecare to provide homecare services as needed. 
  • Demonstrates understanding of participants as unique individuals in need of loving, supportive, emotional, physical, and spiritual care.  
  •   Perform duties in accordance with Code of Professional Nurses.
  • Demonstrates the ability to obtain information and interpret information in terms of patients' needs.
  • Demonstrates knowledge of growth and development.
  • Exhibits appropriate communication skills necessary for ages served (adult/geriatric).
  • Proactively involves the healthcare team in the care of the participant.
  • Attends staff meetings and in-service training as required.
  • Follows all INSPIRA Policies and Procedures and Occupational Safety and Health Administration
  • Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants, and families.
  • Practice Standard precautions and follows appropriate Infection Control procedures.
  • Maintains safe working environment.
  • Participates in and supports the Quality Improvement initiatives.
  • Participates in continuing education classes and any required staff and training meetings.  
  • Maintains professional affiliations and any required certifications.
  • Performs other duties as required or requested.

Knowledge & Skills:

  • Broad knowledge base of health care problems of the frail elderly.
  • Skilled in physical assessment and triaging.
  • Knowledge of common safety hazards and precautions to establish a safe work environment.
  • Skilled in preparing and maintaining records and writing reports and responding to correspondence.
  • Skilled in developing and maintaining department quality assurance.
  • Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers.
  • Ability to react calmly and effectively in emergency situations.
