Job description

The Program Director is a key associate at Hope Network whose primary responsibilities include: Regular and systematic review of service area program operations to evaluate progress towards established goals; evaluates the strategies employed in seeking those goals, and as necessary, implements corrective measures. The position requires little work direction, exhibits persistence and initiative; puts forth a consistent, energetic effort; assumes full and complete responsibility for accomplishments, or lack thereof, for all programs/services within the scope of the Service Area. The Program Director has a high degree of personal integrity, generates positive energy, and makes a difference every day.


ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Regular and predictable attendance is an essential requirement of this position.
  • Ensure that each person served is involved in the individual planning process and has a major role in determining the direction of his/her individual plan. Ensure that the individual plan contains goals and objectives that incorporate the unique strengths, abilities, needs and preferences of the person serve.
  • Ensure the establishment and maintenance of systems to meet all appropriate quality standards and obligations to funders (contracts, etc.).
  • Identify, follow-through, and correct problem areas. Develop and implement methods for improvement of services
  • Planning:
  • Work with Director of Specialized Treatment to develop Service Area short and long range plans and goals to meet Hope Network organizational objectives, consistent with established priorities.
  • Work with Program Managers to set priorities based on consumer needs and resulting in exemplary Evidenced-Based Best Practice service provision.
  • Independant work direction, exhibits persistence and initiative; puts forth a consistent, energetic effort; assumes full and complete responsibility for accomplishments, or lack thereof, for all programs/services within the scope of the Service Area.
  • Work with high degree of personal integrity, generates positive energy, and makes a difference every day.

6. Monitoring:

  • Provide consultation to staff on programmatic matters.
  • Watch closely performance matters concerning consumer outcomes, corporate compliance, service utilization, facility maintenance, chart audits, staff training compliance, and other matters important to achieving program greatness.
  • Consistently monitor progress toward program goals and objectives.
  • Track hospital recidivism and actively work with Program Managers and other staff to reduce unnecessary hospitalizations.

7. Compliance:

  • Validate that monitoring procedures are implemented to detect compliance problems.
  • Ensure that personnel are encouraged to confidentially report suspected fraud and other improprieties without fear of reprisal.
  • Confirm adequate steps are taken to correct any identified compliance problems and prevent the recurrence of such problems.
  • Make certain that 95% of your training and events requirements are renewed prior to expiration date, as specified by job.

  • Notify Program Managers of changes in regulation and standards, as directed.

8. Quality:

  • Ensure internal audits are routinely conducted in accordance to the Hope Network schedule. Likewise, ensure Hope Network' Transparency Model of Practice is upheld. Transparency Model of Practice is being "Audit Ready" vs. Getting Ready" for an audit. It also means directness and openness.
  • Summarize, analyze, and compile monthly reports for Director of Specialized Treatment to review on Service Area's program/service outcomes, fiscal outcomes, administrative outcomes, and HR outcomes. For indicator(s) below benchmark, submit to Director of Specialized Treatment a time-limited and solution-focused plan for reaching targeted outcome(s).
  • Facilitate external audits and validate suitable preparation occurred prior to, thus ensuring auditors have all elements needed to conduct the audit.
  • Ensure 100% of routine/customary daily, monthly, quarterly and/or annual internal data reports are distributed on time.
  • Ensure 100% of contract/grant compliance reporting requirements are completed and submitted by due date.
  • Work toward personal and professional goals and objectives. Attend job related in-services and trainings when offered and actively seek to further develop competencies.
  • Adhere to high levels of health and safety practices.

9. Collaboration/Coordination:

  • Work with internal staff at various levels; keeping information flowing to the appropriate parties vertically (down, as well as, up) and horizontally.
  • Act as a resource for your service area that others can draw upon for new ideas, proposed solutions, and current information.
  • Establish an on-call availability twenty-four hours each day, seven days each week and able to respond in emergency situations.

10. Personnel Development:

  • Recruit and provide direct supervision to a team of professionals.
  • Set goals for own team, develop their capabilities, and maintain a high performance environment.
  • Performance: Maintain good record keeping of staff performance, review appraisal information with staff on consistent basis, aid staff in improving performance on current job, help staff in setting up and implementing job related plans and objectives, cross-train staff.
  • Coaching: Communicate a positive attitude; serve as a catalyst for action and encourage staff to try new things and to take calculated risks; provide honest feedback; minimize tension and defensiveness; create and environment for success; teach and guide staff rather than controlling them.
  • Team Building: Build group cohesiveness and pride; encourage cooperation; recognize individuals and team accomplishments and contributions; share success and rewards; manage conflict, which is inevitable.
    • Monitor team member performance, provide employee counseling, and hold team member accountable to job expectations. Recommend discipline and termination of employee to Human Resource (Personnel), as appropriate.

11. Contract Negotiating:

  • Know the contract requirements for your program / service areas.
  • Support the Director of Specialized Treatment by actively participating on the contract negotiating team.
  • Maintain good relationships with funding source(s) and other key stakeholders.

12. Budget and Economic Management:

  • Compile the Service Area Annual Budget (working with Program Managers), forecasting realistic revenue generation and expenditures, based on programming costs utilization. Submit recommended annual budget to Hope Network Leadership Team, via Director of Integrated Services.
  • Work with Business Manager to forecast annual cash flow needs for Service Area.
  • Ensure alternate funding source(s) are secured through program-driven grant submissions and fundraising activities.
  • Review, on a monthly basis, all service area programs' financial reports and monitor revenue and expenditures to ensure budgetary goals are being met.
  • Consult with, and advise, Program Managers on financial matters related to budget management.
  • Monitor FTE's budgeted, position numbers filled, and management performance related to pay equity.

13. Marketing:

  • Represent Hope Network and serve on community and civic associations, organizations, panels, and/or Boards related to the Service Area.
  • Maintain a professional demeanor.

14. Customer Service:

  • Provide supreme customer service, always being courteous, responsive, professional, and helpful to the organization's internal and external customers.

Addendum for Essential Functions, if applicable for multiple departments:


Harbor Point Intensive Programs:

The Harbor Point Intensive programs offer specialized residential treatment for individuals experiencing mental health and behavioral concerns, and who require a higher level of support and supervision. Individuals served at Harbor Point often have histories of problematic sexual behaviors and/or assaultive behaviors. Residents may also have lengthy histories of psychiatric hospitalizations and/ or criminal histories resulting in periods of incarceration. Assessment of risk and recidivism is conducted prior to placing anyone in the Problematic Sexual Behaviors group, which embraces the Good Lives Model. Psychoeducational groups and therapy groups offered include Dialectical Behavior Therapy (DBT) skills, Acceptance and Commitment Therapy (ACT), Anger Management, Substance Abuse, Cognitive Behavioral Therapy, Seeking Safety, and Social Skills. All services are provided with a trauma-informed approach Regular and systematic review of service area program operations to evaluate progress towards established goals; evaluates the strategies employed in seeking those goals, and as necessary, implements corrective measures.


Addendum for Nursing Responsibilities for Bay Haven Program

  • Licensed Registered Nurse in the state of Michigan
  • BA/BS degree in Social Services from an accredited college.
  • Provide education to staff and residents in all relevant health care related areas as needed.
  • Provide guidance and direction in the dispersion (including medication-passing checks) procurement, and evaluation of individual medications. Monitor the purpose and response to medications, communicating with the physician as needed.
  • Proactively monitor and treat chronic and emergency medical conditions and illnesses within the advisement of affiliate physicians. Develop healthcare goals relating these conditions and provide staff with all appropriate information to carry out these goals.
  • Attend Person Centered Planning meetings as requested.
  • Act as general consultant on medical and health related issues of the participants.
  • Assess participant health needs by performing admission and annual nurse and health assessments. Conduct or insure that quarterly AIMS testing is completed as required. Monitor and/or schedule laboratory tests and examinations in conjunction with program policy.
  • Document medical treatment and health status in resident records for continuity of care and treatment team reference per regulatory standards.
  • Communicate relevant medical issues with all affiliated physicians and applicable program management.
  • Ensure medical compliance with all regulatory bodies (i.e. OSHA, licensing, CARF, etc.).
  • Participate in the development and monitoring of professional nurse services within the Agency.
  • Attend medication review meetings with psychiatrist and/or physicians as designated.
  • Completes face-face-billable services and activity logs in accordance with established productivity rates.

Educational / Talent Requirements:

  • Master’s or equivalent in years’ experience (6 yrs. of related experience).
  • LMSW, LLP preferred.
  • Two years’ experience in human service area, with minimum of 1 year in management.

Work Experience Requirements:

  • Two years’ experience in human service area, with minimum of one (1) year in management.
  • Three years clinical and management experience.
  • Current driver's license with acceptable driving record according to Hope Network insurance carrier definitions.
