The Director of Clinical Liaison Program is charged with the responsibility of leading a team of hospital liaisons who screen potential and existing residents in hospitals, LTACs and any other appropriate medical facility to generate patient admissions and grow hospital accounts.

Responsibilities:

- Create and develop a team to act as liaisons between the facilities and the outside community to safely admit patients.
- Ensure the liaisons have the ability to review residents for admission to the facilities, identify needs and communicate appropriately to the facility teams
- Confirm the liaisons have the capabilities to assess clinical needs of potential/existing residents, and ability to creatively work with facilities to remove barriers to admission.
- Continually asses liaisons performance within accounts and compatibility with the market
- Ability to identify new opportunities in existing and potentially new markets.
- Ensure appropriate coverage of accounts as needed. Track Liaison PTO and shift liaisons as needed to guarantee continual census growth.
- Schedule regular meetings to disseminate information and educate liaison team
- Confirm liaisons are maintaining positive relationships with discharge planners, social workers, hospitals, LTACs, physicians, other nursing facilities and health care agencies to ensure safe transfers of patients.
- Exhibits courtesy, compassion and respect to patients, families, visitors, physicians, administrators and coworkers.
- Attends and participates in departmental/facility meetings, as required.
- Partner with the regional team to remove barriers and safely admit patients
- Prepare and present weekly/monthly reports.
Requirements:
- At least three (3) years’ experience in Healthcare Sales/Marketing/Insurance or commensurate education preferred.
- Experience in Managed Care/Insurance preferred.
- Familiar with Medical Terminology.
- Licensed Practical Nurse; Registered Nurse preferred
