Who is Bridging Access to Care, Inc.: Bridging Access To Care, Inc. is a nonprofit, multi-service, community-based behavioral health care organization.
Since 1986, BAC has maintained an unwavering commitment to serving low-income communities of color. We are Brooklyn's oldest nonprofit prevention, treatment, and health service organization. We provide comprehensive HIV/AIDS, mental health, and substance abuse-related social services to underserved racial and ethnic minority communities in the borough.
JOB SUMMARY: The CORE Coordinator will be responsible for coordinating CORE services, including referral and admission to CORE services, as well as direct service provision. The CORE Coordinator will supervise Program Peer and PSR staff. The CORE Coordinator will receive a combined caseload that may include Psychosocial Rehabilitation (PSR) and Peer Empowerment Services to CORE clients in various community settings or via Telehealth. The CORE Coordinator will model skills and strategies necessary for recovery, thereby developing the client's skills and self-efficacy.
DUTIES AND RESPONSIBILITIES:
- Provide services to help individuals with behavioral health diagnosis/SUD to achieve stability and functional improvement in the following areas: daily living, finances, housing, education, employment, personal recovery and/or resilience, family and interpersonal relationships and community integration.
- Network with and provide community education surrounding HCBS/CORES at Care Management Organizations, Managed Care Organizations, and other treatment providers.
- Outreach to individuals for service from Care Managers and other referral sources
- Conduct HCBS/CORES-specific referral, assessment and intakes with HCBS/CORES candidates.
- Assist in assigning referrals for appropriate services with available staff based on assessment/intake.
- Support the completion of authorization requests, assessments, and ISPs. Work with MCOs to troubleshoot any issues in obtaining necessary documentation and authorization responses.
- Data tracking, including, but not limited to, referrals, outreach, intake, authorizations, admission, goal setting, and reauthorizations.
- Modeling and teaching a person-centered approach, honoring choice, self-determination, and shared decision-making.
- Oversight of the EHR/chart and overall record keeping process.
- Provide individual encounters to CORE clients during home, field visits and/or Telehealth.
- Educate clients on problem-solving skills.
- Engage client in shared decision making around service and recovery planning.
- Review the client's progress toward goals and objectives and update ISP as goals are met.
- Document the client's progress in the eCR system.
- Communicate with other providers such as prescribers, case managers or employment specialists to ensure integrative quality care.
- Work alongside clients as they seek to obtain benefits and entitlements, food, shelter, and permanent housing.
- Raise awareness of existing services and community resources.
- Connect individual to peer run groups in the community and online.
- Advocate on behalf of the client and support them in self-advocacy during appointments.
- Assist client in identifying activities that promote a sense of passion, purpose and meaning.
- Wellness coaching focusing on whole health may include personal sharing of one's own story.
• Employ non-clinical crisis support, especially after periods of hospitalization or incarceration.
- Assist clients in implementing their wellness, crisis plans, or recovery plans.
- Accompany, support and advocate on behalf of client in an emergency or crisis unit if needed.
- Provide short-term crisis support to clients discharged from an emergency room, crisis unit, crisis respite center or incarceration.
- Other duties as assigned and required by BAC
Knowledge, Skills, and Abilities
· Knowledge in trauma informed recovery, both in SUD and mental health.
· Good interpersonal communications skills and the ability to empathize with, relate to and effectively work with participants and peers with chronic illness, substance use and mental illness, or a history thereof.
· The ability to relate to people of diverse backgrounds, cultures, races, sexual orientations and gender identities or expressions.
· Knowledge of community resources for people with chronic medical and behavioral health issues.
· Possesses excellent judgment and the ability to maintain professional relationships and boundaries when working with clients.
· Demonstrates ability and willingness to utilize public transportation to navigate the five boroughs of NYC.
· Experience with and Knowledge of CORE and HARP preferred but not required.
WORK EXPERIENCE:
- MINIMUM: One (1) year experience providing Addiction Counseling
- MINIMUM: One (1) year experience providing Peer Support Services
EDUCATION:
- MINIMUM: Bachelor's Degree
- ∙Must be certified by OMH or OASAS as a Peer Specialist
LICENSURE:
Must be certified by NYS OMH or OASAS as a Peer Specialist
