We have an exciting opportunity at our new Adult Day Health program Seaview in Mashpee, MA for a full time (30 hours per week) ADH Program Coordinator. The schedule is Monday through Friday 9:00 a.m. - 3:00 p.m.
Job Summary:
The ADH Program Coordinator is directly responsible for the development, supervision and management of all aspects of the program.
Duties and Responsibilities:
- Oversee the daily function of the program to include planning, scheduling, coordinating and implementation of appropriate activities, safety and emergency evacuations plans.
- Oversee the implementation of new ideas and activities to increase and ensure program growth and quality services to the participants.
- Maintain and oversee all participant records to include assessment, care plan, progress notes, activity log and medical records.
- Directly oversee and train on-site staff in conjunction with the standards and requirements established by the Department of Public Health regulations for ADH services as well as Council on Accreditation standards.
- Monitor licensing and certifications of all hired staff and consultants.
- Assume the role of coordinator in the referral, intake, admission and discharge processes.
- Coordinate and serve as liaison with individual participant caseworkers, site directors, counseling therapist, medical staff and participants' family.
- Assist in the recruiting, training and hiring of all staff including volunteers and interns.
- Respond to the reporting requirements of the appropriate regulatory agencies.
- Assist in the fiscal administration, including billing, budget preparation and required financial reports.
- Direct the development and implementation of public relations projects for the program.
- Inform participants and their families/caregivers of available community services and make referrals as indications warrant.
- Respond to reasonable expectations of Agency and/or supervisor.
Education and Experience:
- Bachelors Degree in health or human services or a related field preferred with significant work experience with the elderly and/or physically/mentally challenged adults in a community setting.
- Significant supervisory/management and direct care responsibilities with elderly and physically and mentally disabled may be substituted for educational requirement.
- C.P.R. and First Aid certified.
