We are seeking a highly skilled and compassionate Practice Transformation Facilitator to join our health team in Long Beach, CA!
- Up to $70,000 annually!
Primary Responsibilities
The PTF will have the following responsibilities:
Manage a team of Practice Performance Facilitators/Practice Transformation Providers across various practice settings.
Assist and support practices in optimizing available information technology resources to enhance patient outcomes and office efficiencies, covering areas;
Collaborate with interdisciplinary teams to identify opportunities for optimizing information technology resources to support healthcare practices, including:
Population health management strategies
Registry and enrollment functionality, including alignment with relevant programs
Care coordination workflows to improve planned and walk-in care throughput
Implementation of care management strategies to enhance patient care quality and outcomes
Development of patient action plans and goal-setting processes to improve cost, quality, and patient experience
Qualifications
The Practice Transformation Specialist should have the following qualifications:
- Education. Bachelor’s or equivalent degree with a record of strong academic achievement;
- Experience. At least 3 years of clinical practice experience. Ideal candidates will have 5+ years of relevant clinical experience in the healthcare industry; Previous experience in healthcare settings, particularly in primary care practices with successful implementation of quality improvement initiatives, is advantageous.
Pay Details: $66,000.00 to $70,000.00 per year
