Population Health Administrator

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. Job Summary

The Population Health Administrator provides leadership of Population Health management and functions with focus on key areas of integrated care models, complex/chronic care, and community partnerships. Ongoing management and launching of strategic direction of the Population Health for integrated programs and services (mental health, substance use, and medical). Drives the performance of many critical behavioral health care, patient centered medical home, and clinical quality initiatives across the organization. Develops and maintains performance improvement initiatives and ongoing communication with operational leadership, clinical leadership, clinicians and administration regarding productivity, quality outcomes, patient satisfaction, appropriate utilization and all other Population Health initiatives. Directs the application of performance excellence and quality improvement principles and methods to improve organizational performance, including but not limited to analysis of current performance to determine areas of strength and opportunities for improvement, and comparison with best practices and national standards. Responsible for employing practices and performance that are compliant with the confidentiality regulations set forth by the Code of Federal Regulations, Arizona Administrative Code, Arizona Department of Health Services/ Services Provider Manual, Arizona Office of Behavioral Health Licensure, the Commission on Accreditation of Rehabilitation Facilities, and Community Bridges Policies and Procedures.

The Population Health Administrator position will take a lead role with the following agency initiatives:

• ACO data management in collaboration with data analysts, IT and Program staff
• Targeted Investment HEDIS and GIC management, including SDOH initiatives
• VBP contracts: HEDIS, GIC management and attribution/roster management
• Internal Monitoring for GIC, HEDIS, SDOH

• MPH, MHA or MBA or related master's degree required.
• Five or more years-experience in the healthcare industry, including extensive Population Health experience.
• Possess skills in effective training, education, and facilitation of quality improvement efforts by demonstrating up-to-date knowledge and expertise in Population Health.
• Experience with data analytics and technology.
• Experience with health plan relationship management as it relates to Population Health management and operationalizing value-based contracts.
