- Status: Part Time, 10 -20 hours/week
- Hours: 8:30am - 4:00pm
- Schedule: Monday - Friday, flexible scheduling
- $47.94-$83.90 per visit respective to the type of visit/activity, up to 6 visits per day.

Job Summary:
The primary purpose of the Physical Therapist is to provide direct physical therapy to restore patient’s function, alleviate pain, promote safety and self-management of the disease process, and prevent disabilities by planning and administering medically prescribed physical therapy. Job responsibilities must occur in accordance with current federal, state, and local standards, guidelines, and regulations that govern our agency or community, and as may be directed by the Therapy Clinical Branch Manager to ensure that the highest degree of quality care is maintained at all times.
Required Qualifications
- Current Michigan Physical Therapist license; graduate of an accredited Physical Therapy program, who meets the qualifications under the Federal Conditions of Participation for Home Health Agencies as established by CMS.
- Two (2+) years of service of Physical Therapy experience preferred, but will train the right applicant.
- Must have excellent clinical assessment skills as they relate to Physical Therapy.
- Current Michigan Driver’s license.
