- Provide treatment to participants as designed by the PT.
- Consults with PT regarding need to modify treatment plan and goals.
- Measure participants' range of motion and vital signs to determine the effect of treatment.
- May perform standardized assessments to contribute to the PT assessment and/or measure progress of treatment.
- Provide Rehab training for employees, PCA’s, and contracted staff.
- Confers with the Physical Therapist and other members of the IDT.
- Performs skilled modalities.
- Provides training to caregivers, family members and friends related to the functional mobility of participants following their goals and the organizational mission.
- Lead group wellness activities.
- Leads aquatic program and provides interventions to meet participant centered goals for wellness or skilled intervention as indicated.
- Monitors and documents routine maintenance of rehab equipment.
- Assess and provide education and training for participants with braces, prosthesis, splints, diabetic shoes, and other supportive devices.
- Responsible for ordering DME and documenting in the medical record.
- Makes quick repairs for DME as knowledge and skills permit.
- May delegate duties of non-skilled treatment/set-up to PCA Rehab.
- May provide supervision of PTA students.
- Documents in the participant’s medical record as required by policy.
- Utilize the Entrepreneurial Operating System (“EOS Worldwide” by Gino Wickman) business model to drive focus within the organization.
- Must care, support and collaborate with everyone involved in the participant’s care including staff and the participant’s family using the participant centered care model.
- Responsible for completing all required education and file requirements on time.
- Participants in Quality Improvement activities as needed.
- Keeps leader informed of problems or issues.
- Performs other duties as assigned.
- Must possess an Associate’s Degree in the Physical Therapy Assistant Program.
- Must be a graduate of a Physical Therapy Assistant (PTA) Program.
- Must possess a current, unencumbered active license to practice as a Physical Therapy Assistant (PTA) in the State of Michigan.
- Possess current Certified Fitter of Therapeutic Shoes certification by the American Board for Certification in Orthotics, Prosthetics, & Pedorthics (ABC) or obtain certification as offered by Senior Care Partners PACE.
- Minimum of one year of experience working with the frail, elderly, or long-term care population.
- Ability to collaborate effectively with other IDT members.
- Detail oriented with good problem solving skills.
- Strong communication skills, both written and verbal.
