Physical Therapist

At LINK Home Therapy we’re looking to hire a passionate Physical Therapist interested in working in the assisted living setting as a Full-Time employee. Qualifications:

- Graduated from an accredited Physical Therapy program or university

- Valid professional license and registration for state of Michigan

- COVID vaccination mandatory per site requirements

Responsibilities:

- Assist with retrieving scripts/doctor’s orders as requested by DOR

- Complete evaluations, daily notes, weekly progress notes, recertifications, and discharge documentation in a timely manner and in accordance to CMS guidelines

- Establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the physician

- Supervise assistants as requested by DOR

- Collaborates with all disciplines to provide best patient centered treatment and care

- Adhere to scheduled hours and demonstrate professionalism at all times
