JOB SUMMARY:
The Physical Therapist plans and administers medically prescribed physical therapy treatment programs for patients in their residence to restore function, relieve pain and prevent disability following disease, injury or loss of body part. Consults with medical and professional staff to plan and coordinate joint patient management objectives. Acts as a case manager for patients when primary need is for physical therapy. Has contact with patients under a variety of circumstances. Is subject to varying and unpredictable situations and occasionally to irregular hours.

QUALIFICATIONS:*
Education
Bachelor of Science or Master’s degree in physical therapy from a program accredited by the American Physical Therapy Association (APTA).
Experience
Two years of experience as a Physical Therapist. One year of experience in providing rehabilitative therapy to a variety of patients including the elderly. Community/home health experience preferred.
Licensure/Credentials
- Licensed to practice as Physical Therapist in the State of Washington.
- Current BLS certification.
Other
- Valid Washington driver’s license.
- Proof of insurance on automobile used for work.
- Driving record in compliance with OMC standards currently in effect or as may be amended. See HR Policy 3.12 Driving for OMC Business.
- Must have a cell phone (portion of cost reimbursed by OMHH).
- Must have computer internet connectivity (OMHH work laptop provided).
- Must have reliable automobile transportation to travel to patient residences.
