Pharmacist Intern @ Swedish Issaquah

Variable Shift

Per Diem

This position is a pharmacist-in-training and as such may, in the execution of their job and under the direction and supervision of a pharmacy preceptor, perform all the duties of a pharmacist except the final check of medications. An intern (in conjunction with a pharmacy preceptor) is responsible for helping to assure that patients receive safe, efficient, cost effective drug therapy and for providing clinical drug information to physicians, nurses, patients and other healthcare professionals.

  • Interprets and validates orders in regard to legibility and content.

  • Checks patient record for pertinent information before processing order.

  • Evaluates dosage regimen for appropriateness of dosage, interval, formulation and duration of therapy.

  • Prepares, checks and dispenses medications following approved procedures.

  • Makes recommendations to physicians or other health professionals regarding patient-specific drug therapy.

  • Confirms or clarifies patient’s understanding of medication purpose, dosage, dosage frequency and method of administration.

  • Evaluates and monitors therapeutic response to prescribed medications.

  • Demonstrates competency in completing all assigned automated distribution duties and responsibilities.

  • Demonstrates clinical competency with respect to:

  • Appropriate dosing for patient age.

  • Renal function dosing.

  • ADR monitoring.

  • TPN adjustment of electrolytes/insulin.

  • Therapeutic interchange.

  • PIP documentation.

Required Qualifications:

  • Currently enrolled in an accredited school of pharmacy.

  • upon hire: Washington Pharmacist License. Or

  • upon hire: Washington Pharmacist Intern Registration.

Preferred Qualifications:

  • Experience in healthcare environment.
