Hospice East Bay provides compassionate end-of-life services to terminally ill patients in our community, while offering emotional, spiritual, logistical and bereavement support for their families and caregivers. Essential Functions
- Reviews pre-admission documents from the attending physician before arrival to the setting along with preparing and organizing chart paperwork
- Follows admission visit design format for consistency and accurate, timely completion of evaluation or admission process
- Inquire about patient and family preferences for palliative vs. curative care and reviews criteria for admission
- Educates patient and family about full range of Hospice services and the interdisciplinary team approach
- Provides Hospice Information Handbook and discusses key sections; provides other literature as appropriate
- Completes an initial assessment and discusses plan of care for visits, medications, when to call Hospice, safety, etc. and explains consents, other documents and obtains signatures
- Completes documentation by the end of the working day
- Provides report to care team members
- Collaborates with contracted agencies for DME, medications, other approved services as well as the patient's attending physician and the HEB physician
- Responsible for 2 admissions daily
- Provides consultation to patients and families in home/hospital regarding hospice services including if appropriate for Bruns House (GIP)
- Flexible schedule to ensure admissions are completed the same day as the referral
- Demonstrates sensitivity to culturally diverse patient, family and employee populations
Position Requirements
- Demonstrates the skills, knowledge and abilities necessary to be successful in the job as outlined in the position requirements
- Excellent interpersonal and communication skills
- Knowledge and understanding of the hospice philosophy and hospice criteria for admission and the admission process
- Knowledge of private insurance, Medicare and Medi-Cal regulations as well as State and Federal regulations
Working Relationships
- Ability to conduct yourself professionally in different circumstances as a potentially fist representative for HEB
- Able to work well under pressure, meeting multiple and sometimes competing deadlines and changing priorities
- Demonstrate cooperative, professional behavior with colleagues and clients.
- Flexible with work assignment
- Adhere to agency as well as departmental goals and performance standards.
- Participates in agency clinical meetings as requested
- Presents with professional office attire or uniform
- Committed to principals, practices and procedures of nursing in a hospice environment
Experience & Education
- Valid California registered nursing license
- California driver's license and insurance
- Minimum of five years of nursing experience
- 1 year RN experience within the last 3 years
- Hospice or home care experience preferred
Job Types: Full-time, Part-time
Pay: $55.00 - $63.00 per hour
Expected hours: 24 – 40 per week
