Position Summary Pediatric – Mother/Baby Social Worker

At Stony Brook Medicine, our Social Workers are valuable members of our team, who provide empathic care to our patients, their families and visitors, with the highest degree of integrity and respect. Social Workers apply the principles and practices of their profession to provide integrated social casework, including direct counseling, therapy, advocacy, and other services to individuals, families and groups in a variety of settings. Social Workers plan, implement, coordinate and document treatment for individuals diagnosed with: medical/ surgical acute care needs, behavioral health issues, intellectual and developmental disabilities; work-related illnesses or injuries; substance use disorder and addiction; or perpetrators or victims of neglect, abuse, or crime who need therapeutic intervention, habilitation, training, supervision and re-socialization, or confinement.

Qualified candidates will demonstrate excellent communication skills, interpersonal skills, knowledge and understanding of patient care and effectively respond to changing patient needs by making decisions based on ethical principles and adhering to our high standard of excellence.

Duties of a Pediatric Mother/ Baby Social Worker may include the following but are not limited to:

  • Psychosocial assessment of selected cases.
  • Effectuate discharge planning.
  • Coverage Inpatient Pediatrics, Pediatric ICU, Pediatric Oncology, Mother/Baby, Neo-natal ICU
  • Attend rounds and coordinate with medical team.
  • Work with community agencies in regards to patient post discharge needs.
  • Work with SB Outpatient division for improved continuum of care.
  • Compliance with documentation of Social Work intervention.
  • Coordination of discharge transportation, insurance verification, advocacy with insurance companies for discharge planning and insurance reviews as needed.
  • Assist patient financial services, benefits or with Medicaid applications as needed.
  • Coordinate and give review to insurance companies as needed; contact insurance companies for authorizations.
  • Assist patients and families with in hospital support and d/c planning
  • Assist with care giver support
  • Act as mandated reporter and coordinate with medical team on patients with suspected neglect, maltreatment or abuse
  • Create plan of safe care for new mother’s and parents involved with Substance Use Disorder
  • Ability to cover Neo-natal ICU
  • Supervision of professional social work students.
  • Active involved in performance improvement activities including program development, data collection and analysis with Care Management Analytic team.
  • Coordinate with Psychiatry and Psychiatric Social work, peers, and addiction counselors for behavioral health patients on medicine units.
  • Discharge planning for PEDS specific discharges (peds/infant rehab, acute care placement, behavioral health placement, Inpatient psych placement as applicable)
  • Cross coverage on units.
  • Coverage in outpatient clinics as needed
  • All other duties as assigned.

Qualifications:

Required: MSW from an accredited Social Work Education School. New York State LMSW or LCSW. Three years post Master’s in an inpatient medical health care setting. Demonstrates excellent written and oral communication skills. Demonstrates strong Psychosocial Assessment skill set. Demonstrates understanding of discharge planning and long length of stay barriers and systems. Working knowledge of Long Island and Metropolitan NY Community resources. Demonstrates patient first, patient advocacy and service excellence. Computer literate and understanding of data analysis.

Preferred: Experience in Pediatrics and Mother/Baby Social Work; Bi-Lingual; Ability to cover other medical units.

  • Provide a copy of any required New York State license(s)/certificate(s).

Anticipated Pay Range:

The starting salary range (or hiring range) for this position has been established as - $87,885 - $105,031 Base

The above salary range (or hiring range) represents SBUH’s good faith and reasonable estimate of the range of possible compensation at the time of posting.

In addition, all UUP positions have an additional $3,087 for location,

Some positions offer shift differentials for Night ($15,000) & Evening ($10,000) as well as $575 for inconvenience for positions not on day shift. Please visit these links to learn more.
