Job Description

Hours: M-F 9:00a.m. to 5:00p.m. (2nd Saturday of the month 10-2pm)

Department: Pediatrics and Adolescent Medicine/YHEP

(Located at 1207 Chestnut Street., 5th Floor)

Admin Supervisor: Pediatrics/YHEP Practice Manager

Clinical Supervisor: Director of Nursing

Full-Time, 40 hours per week; Exempt

We are seeking a passionate and motivated Nurse to work in Philadelphia FIGHT's pediatric Young Adult health centers. This is a full-time position at 40 hours per week. Philadelphia FIGHT, initially founded in 1990 as a comprehensive AIDS service organization, is now a Federally Qualified Health Center with four clinical sites, providing access to state-of-the art, culturally competent primary care, advocacy, social services, consumer education, and outreach for all patients in need, regardless of HIV status.

Philadelphia FIGHT Pediatrics and Youth Health Empowerment Project Health Center (YHEP) together provide comprehensive and culturally effective health care to children and Young Adults from birth to 26 years old, with a particular focus on immigrants and LGBTQ+ children and youth.

The patients and staff at Philadelphia FIGHT are diverse, representing a broad spectrum of race, religion, gender identity, sexual orientation, HIV status, and physical ability. As such it is imperative that candidates demonstrate sensitivity to the needs of various populations.

We are looking for a Nurse who is committed to exceptional public health care provision, and someone with a specific interest in working with Spanish-speaking immigrant populations as well as with the trans and gender non-conforming community.

Job Responsibilities

Clinical (0. 7 FTE)

  • Triage walk-in patients, sick patients and sick telephone calls and electronic messages
  • Communicate normal and abnormal labs to patients, as requested by the medical provider
  • Provide teaching, as needed, to patients with chronic conditions, such as asthma and diabetes
  • Provide education and counseling, as needed, to patients initiating gender affirming hormone therapies
  • Utilize EMR (eClinical Works) for documentation of all medical care provided to pts while at the clinical site as well as when providing care "behind the scenes"
  • Support Medical Assistants when needed by preparing patients for their visit with vitals and age-appropriate screening tests, administering therapeutic injections and vaccinations and performing phlebotomy
  • Perform point of care tasks such as blood glucose monitoring, urine dipsticks, nebulizer treatments and pregnancy testing
  • Perform in-house procedures including wound care, ear lavages, EKGs and other in-house procedures
  • Perform CLIA related tasks such as running of controls
  • Monitor clinic /patient flow to ensure timely care and completion of multi-service appointments
  • Demonstrate commitment to excellent inter-disciplinary clinical care and confidentiality

Administrative (0.3 FTE)

  • Assist providers in the scheduling of imaging studies and procedures requiring prior authorization
  • Support daily clinical huddle with providers, nurses, medical assistants and other support staff
  • Identify, track and follow up on high-risk patients to ensure appropriate follow up, medication utilization,
  • Collaborate with the behavioral health consultant, PrEP staff, Medical Case Management and medical providers to ensure that high risk patients are meeting their treatment plan goals
  • Order and coordinate delivery for durable medical equipment
  • Report and assist in communicating appropriate information on reportable diseases to oversight bodies, g., Public Health Department.
  • Coordinate ordering of clinical supplies and samples including monitoring for expiration of supplies and maintenance of clinic medical equipment (g., medical supplies and prescriptions)
  • Participate actively in Quality Improvement initiatives and committees
  • Participate in regularly scheduled practice-related meetings and attend FIGHT case conferences
  • Assist and support outreach and recruitment initiatives, in addition to educational programs
  • Demonstrate commitment to clinical care and confidentiality
  • Work as a team with medical and administrative staff
  • Complete all charting and documentation as needed and required by contracted funding sources, including assessments, progress notes, and administrative

Qualifications

Education: Bachelor's Degree preferred

Certification:

  1. Registered Nurse
  2. Within 30 days of hire, complete training in Blood borne pathogens, Post exposure prophylaxis, and HIPAA
    1. Prior to shipping blood specimens, complete IATA training for shipping of hazardous substances, if applicable
  3. General knowledge base in science, medicine, and the running of a medical clinic. Demonstrate the ability to understand the overall purpose of clinical care and importance of support staff.
  4. Excellent interpersonal skills and ability to work collaboratively with primary care staff, patients, and other behavioral health counselors. Must be able to demonstrate judgment, patience, integrity, organization, and prioritization in multi-task. Needs to have attention to detail, dependability and flexibility.
  5. CPR certification required within 60 days of employment

Work Environment:

  • Generally, works in a clinical office environment but may occasionally be required to perform job duties outside of the typical clinical office

Initial probationary period is three months. Health benefits begin on the first day of work and additional benefits are offered per the employee handbook.
