Job Summary:
The Pediatric Care Coordinator is a core member of the Pediatric Care team and Pathways department. They provide one on one office visits with families providing education, support, referral assistance, and assist with individual needs that the patient might need. They work directly with pediatric providers and nursing staff.
Essential Functions
- Perform office visits and ongoing phone contact with pediatric patients to assist families with unmet needs and provide education on additional NorthShore and Community services. Provide education on child development, parenting, nutrition, health/wellness, safety, and other topics deemed necessary.
- Gather health information from patient and family to educate them on health-related needs and assist with scheduling additional health and wellness services. (including but not limited to well-child topics, immunization, annual physicals and screenings, dental exams and cleanings, vision exams, behavioral health etc.)
- Promote NorthShore services to patients and families.
- Education patients on enrollment options for Medicaid, Marketplace, and signing up for slide scale.
- Provide assistance and support during Outreach events.
- Complete and maintain timely and accurate documentation in EMR programs.
- Meet department and company goals.
- Other related duties assigned by Pathway Program Manager.
