Position Description
The Patient Navigator is the 'front line' personnel who will act as a patient advocate working with the patient care team in communicating scheduling issues. The position will assess the patient's needs, assist physicians and nurses with moving patients through the continuum of care by scheduling patient appointments across several sub-specialties, making appropriate referrals to provide navigation and guidance, and navigate all criteria-based scheduling.
Position Requirements
Associates Degree in Healthcare or related field and 2 years or more of experience in a healthcare setting, patient scheduling or care coordination. Additional years of relevant work experience and/or other specialized training can be used in lieu of minimum education requirements. Computer literacy, excellent organization, and communication skills.
