Position Title:Patient Care Manager - Care Center
The Patient Care Manager plans, directs and coordinates the hospice clinical program, inPatient Care and nursing home/ACLF program for multiple teams. This position works in an interdisciplinary care setting to teach and mentor Patient Care personnel. The PCM coordinates all interdisciplinary functions and ensures that patients' and families' needs are met and participates in the on-call rotation as necessary. Serves as member of Haven's management team.

You Have:

• Bachelor's in Nursing or a related field of Healthcare required. Experience will be considered in lieu of degree.
• Licensure & Certifications- Current Florida RN license without restrictions.
• Requires a current Cardio Pulmonary Resuscitation (CPR) certification (American Heart Association).
• Hospice and Palliative Care Certification preferred. Florida driver's license and auto insurance required.
• 1 year of managerial experience required and Hospice experience strongly preferred.

Location:Lake City - Care Center
The Lake City Care Center, opened in January 2007 in Lake City. This 16-room facility serves patients and families from six counties in the Suwannee Valley area. The Care Center is located at 6037 W US Highway 90, Lake City. Lake City is surrounded by the Sante Fe, Suwannee and Ichetucknee rivers and numerous springs, the sheer beauty of the landscape inspires families to grow and thrive while the flourishing economic environment attracts a robust healthcare climate, plenty of shopping and top notch education systems from primary to post secondary, creating the ideal balance.
