Job description

Job Summary:
The Patient Care Coordinator is accountable and held responsible for managing the clinical care and course of treatment for a number of patients. Coordination of care is accomplished through collaboration with physicians, associate staff, and other health care providers to ensure that patient outcomes are achieved within the projected course of treatment using the nursing process. The Patient Care Coordinator is accountable to the Nurse Director. They will utilize an appropriate knowledge base and skill set to communicate and interact with the patients, families, and visitors of all age groups while being sensitive to their cultural and religious beliefs. The coordinator will function as an extension of the Director with oversight for the practice of nursing on this unit.
Performance Expectations:

  • Meets established standards of practice of assessment and reassessment from triage to discharge/admission/transfer as evidenced by appropriate documentation according to policy and procedures, protocols, patient satisfaction.
  • Competent in patient interventions as evidenced by safe administration of medications, treatments, procedures, infection control, and addressing changes in status to safely care for patients
  • Utilizes the knowledge and skills to safely and appropriately fulfill job specific responsibilities related to critically ill patients using evidence based practice
  • Works the ED results list in EPIC and follows up to make sure appropriate care is given.
  • Assists director with evaluations of staff
  • Responsible for patient flow in the ED
  • Responsible for making staff schedules
  • Will hold staff accountable for maintaining a safe, quiet, and efficient workplace.
  • Works closely with house supervisor to ensure efficient flow for admits from the ED to the inpatient units.
  • Responsible for making staffing assignments and monitoring breaks and lunches.
  • Utilizes critical thinking abilities and possesses the ability to care for the Trauma, Stroke and STEMI patient according to protocol, policy, and procedures.
  • Utilizes the ability to care for the cardiac patient and identify lethal cardiac dysrhythmias.
  • Utilizes the ability to perform cardioversion, defibrillation and transcutaneous pacing.
  • Utilizes the ability to assist with set up and maintenance for sterile field during procedures.
  • Utilizes the ability to assist with cardioversion for atrial tachycardia, atrial flutter and atrial fibrillation.
  • Competently performs peripheral IV insertion and removal and initiates IV therapy in accordance with standards, policies and procedures.
  • Utilizes the ability to perform moderate sedation.
  • Utilizes the ability to perform as a Triage nurse.
  • Utilizes the ability to collect evidence, following all state, federal and/or hospital regulations.
  • Keeps a clean, safe, quiet, and efficient work environment.
  • Utilizes the ability to communicate effectively with staff, managers, physicians, and executive team

Qualifications:
Education/Skills
Graduate of a recognized non-online RN program. Basic Rhythm Interpretation and EKG training within 1 month of hire. NIHSS required within 1 year of hire. External Juglar training and competency must be completed within 1 year of hire. Moderate sedation training during orientation period.
Work Experience:
Minimum of two years as a staff RN in an acute setting preferred.

Utilizes ability to understand and use appropriate coping mechanisms in dealing with death and dying
