Pediatric and Adolescent Transformative Healthcare (PATH)
The Pediatric and Adolescent Transformative Healthcare (PATH) Collaborative will provide an innovative structure for a Behavioral Health Home (BHH) serving children, adolescents, and families on the west and southwest sides of Chicago. The purpose is threefold:
(1) To expand access to developmental health providers to conduct assessments and provide services including occupational, speech, and physical therapy
(2) To expand access to pediatric behavioral health providers to address the growing mental, emotional, and behavioral health needs of children and adolescents
(3) To address the social and structural determinants of health through care coordination, case
management, peer support services, home visiting, policy and advocacy, localized hiring, workforce development, data sharing, and collaborative learning.
PATH is led by Saint Anthony Hospital, a community-based safety net hospital, and includes federally qualified health centers (FQHCs), an inpatient pediatric behavioral health hospital, community-based organizations, a Business Enterprise Program (BEP) population health tech company, a BEP fiscal intermediary, and an evaluator specializing in assessment through a racial equity lens
CCCS/Family Focus, a leader in the field of family support, seeks qualified candidates for the full-time position of Parent Educator. Family Focus provides comprehensive resources and support programs for families with young children and at-risk youth at multiple community-based centers in Chicago and the Chicago land area. Founded in 1976 by early childhood professionals, Family Focus provides services to a diverse population.
DUTIES AND RESPONSIBILITIES
- Meet regularly with PATH Supervisor to discuss and review cases.
- Maintain required screenings, updated immunizations, educational status, WIC appointments, and family planning for participants and their children.
- Attend and participate in all meetings, conferences and in-service trainings required by Family Focus.
- Prepare for and Facilitate groups at center and collaborating organizations as needed.
- Participate in all center wide trainings, activities, and meetings.
- Work with the other PATH agency partners to address the needs of the participants served.
- Work in a team environment.
- Recruitment and outreach.
REQUIRED SKILLS AND QUALIFICATIONS
- Minimum of a Bachelor of Arts degree (B.A.) in Human Services, Early Childhood Education, or a related field. Or have 5 years of experience in the social service field.
- Be fully Bilingual and have experience working with multicultural participants.- preferred
- Experience in child development, early childhood education, social work, or related field.
- Demonstrated ability to communicate clearly both verbally and in writing.
- Knowledge and understanding of community.
- Requires extensive local travel and some evening and weekend hours.
- Must have access to reliable and efficient transportation that will allow for the completion of caseload home visiting responsibilities.
Pay $46,800/year
