First Choice Home Health & Hospice is seeking to hire a Part-Time RN to care for our Home Health & Hospice patients in Salt Lake County. Visit types include admit oasis, high & low acuity skilled follow-up visits, wound care, patient assessments, and care team coordination. Work is performed directly in the homes of the patients (see below for more detailed duties).
Pay per visit - $50-$100 plus mileage

Active RN license in good standing
Valid Driver's license
Reliable transportation
More Detailed Duties & Responsibilities:
- Complete general assessment of patient including:
- head to toe assessment
- assessment of wounds
- safety evaluation
- develop plan of care
- head to toe assessment
- Make follow-up visits to patients including:
- checking vitals and comparing them to individual thresholds
- taking blood and urine samples, and delivering them to lab
- dressing changes
- checks blood sugar and gives insulin to diabetic patients.
- educate patient and care givers on what can be done to improve status
- Coordinate with care team
- report findings to MD as appropriate
- create CNA care plan and provide oversight
- work with care givers and therapists to reach patient goals and outcomes
- keep family members informed as appropriate
- develop discharge plan with team members
- Attend team meetings
Percentage of Responsibilities:
Direct clinical care – 80%
Documentation and coordination - 15%
Attending meetings – 5%
