The Home Health Scheduler works closely with the company’s clinicians creating and maintaining their schedules. They ensure adequate patient coverage and scheduling of new patients as they are admitted to our Home Health care. Works with the Executive Director and Director of Nursing to manage case loads.
- Creates and maintains Home Health staff visits according to care plans and staff availability, including accommodating for sick leave, PTO and schedule changes.
- Builds strong relationships with all staff and acts as a liaison between them and the company.
- They perform a variety of office tasks including but not limited to filing documents on behalf of the company, both physically and electronically, organizing and coordinating meetings, and ordering supplies both for the company and for patients as needed.
- They work within the company’s Electronic Medical Records (EMR) system.
- Completes assigned training on time.
- Maintains patient confidentiality at all times.
- Performs other duties as assigned.

High School or College Education.
HomeCare HomeBase experience required.
Business and professional setting experience.
Health care experience preferred.
