JOB DESCRIPTION: The Community Mental Health Promotion and Support (COMHPS) Outreach Worker is responsible for Community Mental Health counseling and support services to individuals, families, and groups seeking help coping with stress and seeking advice about social support and behavioral health resources.
SPECIFIC RESPONSIBILITIES INCLUDE:
· Works with individuals, families, and groups to provide Community Mental Health counseling and support services either virtual or in-person that includes outreach, wellness activities, assessment, referral and linkage activities, brief individual and group counseling, family education and counseling and academic and vocational support.
· Completes all necessary program documentation and assessments when working with clients.
· Completes all community based service provider trainings before beginning any service delivery.
· Represents program in the community and networks with other agencies and partners to ensure needs of clients are met.
· Provides presentations to community groups on disaster reactions, coping skills, stress management, and the COMHPS Program.
· Participates in daily and weekly meetings and de-briefings with the Team Leader.
· Participates in training activities coordinated by the OMH Central Office oversight team and the OMH Center for Practice Innovations.
· Must have valid Driver’s license and transportation
· Maintains self in a professional manner at all times, adhering to Neighborhood Center policies, procedures and mission.
· Must have flexibility to work non traditional hours; including night and weekend outreach events as identified by the Program Director and team outreach calendar.
· Performs other duties/tasks as assigned.
REQUIRED QUALIFICATIONS:
· Possess the capability to provide services in a caring manner within the parameters of the COMHSP Scope of Work. Demonstrated extensive knowledge of community resources and multiple human service delivery systems.
· Demonstrated organizational, assessment and communication skills commensurate with this position.
· Demonstrated ability to be flexible and work independently.
· Demonstrated ability to secure cooperation of and work effectively with supervisory staff of other agencies in the community.
· High School Diploma/Equivalency Diploma and one (1) year human services experience preferred.
