Description

GENERAL PURPOSE:

Under the supervision of the Chief Clinical Services Officer (CCSO), this position is responsible for providing coordinated care for patients for various clinical screenings and testing. Performs HIV and Hepatitis C antibody testing, STI screening, counseling, and referral services.  

MAIN RESPONSIBILITIES AND DUTIES:

  1. Works to coordinate the care for patients who are at high risk for Hepatitis C in addition to testing for HIV and sexually transmitted infections to ensure that they have prompt access to any necessary care including our providers, mental health, specialty care, etc. The employee will link patients to HIV treatment, health information, referrals, risk reduction interventions, condom distribution, STI awareness, testing and treatment, other social service referral, and PrEP/PEP services.
  2. Assists patients in receiving medical care for HIV, STI, and Hepatitis C infection and other support services.
  3. Works closely with Ryan White Team as it relates to counseling for positive results.
  4. Maintains excellent communication with the patients and the care teams.
  5. Contacts patients to schedule preventative screenings, appointments, and schedules transportation if needed. 
  6. Assists the patients with needed health interventions.
  7. Check-ins on the patient regularly, evaluates, and document their progress.
  8. Counsel the patients to discuss their health status.
  9. Educates patients about their condition, medication, and provides them with specific instructions.
  10. Develops a care plan to address their personal health care needs.
  11. Utilizes strong organizational skills necessary to systematize care for several patients at once, while coordinating with diverse healthcare workers.
  12. Ensures highly developed verbal and written communication skills to expedite and facilitate patient understanding of health needs while maintaining a high level of respect and confidentiality for the patient.
  13. Consults and collaborates with other health care providers and specialists to set up patient appointments and treatment plans.
  14. Communicates with others throughout the organization and community to encourage coordination in patient care.
  15. Collects data through patient tracking to facilitate patient outcome data collection and analysis.
  16. Ability to work in a fast paced, patient service-oriented environment.
  17. Maintains professional growth by attending trainings, staff meetings, and required in-services as necessary. May be asked to attend the State Office of AIDS HIV Test Counselor certification program.
  18. Treats patients with empathy and respect and conducts oneself in a professional manner working with patients who may use drugs, have a mental illness, or other chronic difficulties.
  19. Complies with organizational guidelines and health care laws and regulations.
  20. Will attend outreach events and street outreach work for testing and screenings.
  21. Will visit all clinics to provide point of care testing for patients already in the clinics.
  22. Adheres to strict procedures of confidentiality, the release of information, and data security.
  23. May be exposed to Bloodborne pathogens.
  24. Conduct weekly, monthly, and quarterly reports perform data entry and provide monthly documented patient follow ups.
  25. Maintain accurate and current process, procedural, and program, and inventory logs. Orders supplies as needed to be prepared for testing and screenings.
  26. Provides and conducts PrEP and HIV navigation services.
  27. Runs monthly HIV testing controls.
  28. Orders HIV, STI, and HCV education and prevention materials.
  29. Makes sure specimens are collected properly.
  30. Other duties as assigned by supervisor.

PROFESSIONAL KNOWLEDGE, SKILLS & ABILITIES:

 

  1. High School Diploma
  2. Completion of Medical Assistant or phlebotomist
  3. Ability to problem-solve utilizing the team approach. Conflict resolution skills
  4. Possesses outstanding interpersonal skills, including excellent written and verbal skills. Must have computer skills using Microsoft software applications such as Word, Excel, Outlook, and PowerPoint.
  5. Ability to deal with people in person and in telephone contacts in a professional manner. Communicates effectively with patients for appointments and follow up on referrals through culturally competent and trauma informed approaches.  
  6. Demonstrates ability to promote testing and screenings meeting quotas.
  7. Develops schedule for daily testing and screenings.
  8. Sensitive to the needs of the cultures represented in the corporation’s patient population.
  9. Demonstrate clear knowledge of Ampla Health’s clinic structure, standards, procedures, and protocols.
  10. Experience and knowledge of the culture and environmental issues significant to people of color, immigrants, high-risk youth, low-income families, people who inject drugs and other high-risk drug users, individuals involved with the criminal justice system, homeless and unhomed, patients with chronic severe illness, and mental health issues.

COMMUNICATIONS SKILLS:

  1. Must have neat and legible handwriting.
  2. Must be able to interact with patients courteously and calmly.
  3. Ability to communicate well with the public.

WORKING CONDITIONS & PHYSICAL REQUIREMENTS:

Works will with patients in a generally comfortable environment office. Employees must possess the following physical requirements:

 

  1. Must be in good health and able to lift up to 40 lbs.
  2. Must be able to hear staff on the telephone and those who are served in person, and speak clearly to communicate information to clients and staff
  3. Ability to operate standard office equipment such as a computer, telephone, fax machine, copier, etc.
  4. Must be able to reach above the shoulder level to work, bend, squat and sit, stand, stoop, crouch, reach, kneel, twist, lift, and turning, as well as walking up to a mile several times a day, climbing stairs, and being comfortable in street settings throughout the communities where the program’s patients may live.
  5. When using a company vehicle performs fueling and routine minor maintenance for the vehicle keeping it neat and clean. The employee may be asked to use their own vehicle for travel when a company vehicle is not available. Therefore a reliable vehicle with appropriate automobile insurance, a valid California driver’s license, and a safe driving record is required.
