Job Description

POSITION: Outreach Assistant

DEPARTMENT: Fund Development and Community Outreach

CLASSIFICATION: Non-Exempt –Part Time (20 hours per week)

SUPERVISES: N/A

ORGANIZATION OVERVIEW

The mission of Alameda Point Collaborative (APC) is to end homelessness by creating permanent supportive housing and trauma-informed services where homeless individuals and families can flourish. Established in 1999, APC grew out of the collaboration of 20 agencies to transform the decommissioned Alameda Naval Air Station to alleviate homelessness in Alameda County. APC gained a federal conveyance to re-use 34 acres of land, buildings, and former military residences into 200 supportive housing units for homeless families, plus offices, parks, and agricultural space. In addition to providing stable housing, APC offers the following wrap-around services: case management counseling, mental health recovery, and assistance in achieving resident-set goals towards self-sufficiency; life skills development, job training, and placement; children and youth programs to encourage healthy social, academic, and emotional development; food security initiatives, consumer awareness, and financial acumen through classes and opportunities at our three social enterprises: Ploughshares Nursery, APC’s Farm2Market, and APC’s Commercial Kitchen.

FUND DEVELOPMENT & COMMUNITY OUTREACH

Alameda Point Collaborative is primarily funded by government grants with the stipulation that a 25% ‘match’ is raised by APC. The match may be raised from individuals, foundations, or corporations. In addition, in-kind gifts of goods or services and volunteer hours may count towards the match. The fund development team is responsible for facilitating gifts through annual appeals, online asks, one major event and community initiatives, writing grants, and making proposals to businesses and individuals. In addition, the team coordinates the APC website, social media, and community outreach initiatives.

ALAMEDA WELLNESS CAMPUS (AWC)

APC and its partner agencies, LifeLong Medical Care and Mercy Housing, are creating the Alameda Wellness Campus. The AWC will create one of the nation's first centers with integrated services for unhoused adults with complex health conditions. The 3.6-acre campus is adjacent to East Bay Regional Park’s Crab Cove in Alameda, a beautiful shoreline area providing a healing environment for unhoused, medically vulnerable Alameda County residents.

The AWC will co-locate:

  • 100 permanent supportive housing units for unhoused Alameda County older adults
  • 50-bed high-acuity medical respite program for unhoused Alameda County residents to provide recuperative care for homeless clients who require medical care after hospital discharge or who are referred by street outreach teams
  • Health clinic to provide medical, behavioral health, palliative care and wellness services
  • Resource Center to provide case management, housing prevention, and housing placement services for homeless and vulnerably housed City of Alameda residents

The project intends to enhance the standard of care for a highly vulnerable subset of the homeless population with significant medical, geriatric, behavioral health, substance use disorder, and end-of-life care needs. The AWC will serve an estimated 700 unhoused Alameda County residents annually.
