SHOPP has several service programs thatincludesCommunity Care Network Program (CCNP);VIPElder Justice;NYConnectsoftheBronx;Case Management & Friendly Visiting Program; Caregivers Program, Lafayette Estates NORC Program; Older Adult Centers: Casa Boricua, Leon, Soundview, and Guess.
SHOPP Caregiver SupportProgram (CSP) supports formal and informal caregivers caringfor anill, frail, disabled or cognitivelyimpaired individual withAlzheimer's orother forms ofDementia; or those 55plus Kinship relation- grandparent raising a grandchild(ren).TheOutreach and Digital Coordinatorwill be responsible forperformingintakes on callers/ walk-ins seeking services and support, overseeoutreach initiatives in the community to generate client referrals as well as promote events, messages and content to help engage clients and potential partners.
Services to be Rendered:
TheOUTREACH and Digital COORDINATORis specifically responsible for the following:
- Complete Caregiver Intakes and preliminary assessmentson callers seeking information, support,and services.
- Coordinateweeklyvirtualoutreach initiativesto generateand identifyclient referrals, educate communities,buildlinkages and contactsfortheprograms, recruit volunteers, and identify organizations able to host and support our efforts in the local community.
- Followoutreach plan to promote the program and its services.
- Generate leads of community partners to hostSHOPPservices through face-to-face meetings or virtually due to pandemic.
- Conducts research to create informed and attractive content that appeals to a target audience of potential customers via Social Media, Newsletter, Website. The content is placed in formats that can be used on various channels and reflective to the diverse communities we serve.
- Work with SHOPP program staff to develop and promote events, messages and content to help engage clients and potential partners.
- Create postings, marketing and advertisement material that can be shared to communicate with consumers. The postings should streamline the brand and reputation of SHOPP.
- Responsible for following any conversations relative to the post and to interact with any public responses. Ensure you maintain the integrity of the SHOPP brand.
- Enhance vision and SHOPP brand by posting agency appropriate pictures, text, videos and other content used to draw attention to the brand or topic.
- Set goals to track the efficiency of campaigns / content and compare similar campaigns / content to ensure it is reaching the intended target of customers/consumers.
- Cultivate relationships and manage constant contact list to ensure potential partners are placed on list.
- Be available to provide support to the Caregivers Program team by effectively moving prospects clients through the process, when needed.
- Build relationships and visit community agencies, churches, institutions, and other community locations to generate referrals, promote our services, and explore partnerships and cross-promotion opportunities.
- Coordinate opportunities to presentvirtualworkshops, presentations and/or activities.
- PromoteSHOPPservices in the community using local resources (i.e. bulletin boards), Community Boards, local media, and the internet.
- Attendmonthlysupervision and team meetings.
- Participate in and representSHOPPin community fairs and events.
- Log and track units of serviceas mandated by SHOPP and NYC AGING.
- Maintain an accurate Worker Log and other supporting documentation.
- Effectively utilize social media and other digital and communication tools to reach a broad audience.Follow and subscribe to professional development resources (i.e.e-newsletters, professional social media, national aging websites, etc.).
- Adhere to NYC AGING and other relevant government guidelines, regulations, procedures,and compliance expectations.
- Comply with SHOPP Personnel Policy.
- Communicate with supervisor and other staff in a timely fashion and meet all required deadlines.
- Perform other tasks as assigned by the Program Director.
Minimum Qualifications:
The OUTREACH/DIGITAL COORDINATORwill report directly to the CSP Program Director. Bachelor's degree in relevant field required such as Social Work, Communication, Psychology, Public Health etc. with good Public Speaking skills. Experience in sales and marketing preferred. Must have excellent Interpersonal and Organizational Skills; Office Management and Computer Skills and
excellent communication skills (written and oral). Bilingual fluency (English/Spanish) required due to population we serve. Must be self-motivated, organized and professional with an ability to work independently and with the interdisciplinary team. Must demonstrate maturity, sensitivity and good judgment in relating to peers, partners, caregivers and care receivers. In addition, must demonstrate the ability to work with people from diverse backgrounds. Availability, dependability, and willingness to be flexible will be a priority in meeting the needs of the various types of caregivers we serve. Demonstrated ability to problem solve and think strategically will be vital.
