As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of Rochester, NY and in Canandaigua, N.Y.
Jordan Health is now seeking an OB/GYN Social Worker who, under the guidance of the Director of Care Coordination, is primarily responsible for coordinating services to meet, social, emotional, and medical needs of patients.

Requirements
The OB/GYN Social Worker will:
Demonstrate the ability to identify, assess, counsel and effectively intervene with medically and psychosocially complex patients with the ability to understand complex dynamics related to the clinical and social situation.
Provide crisis intervention services to patients who are expecting crisis and provide short term care management.
Successfully utilize counseling methods such as crisis intervention, psychosocial support, brief goal-directed therapy, problem solving methods that may be appropriate to the clinical situation.
Continues to monitor and evaluate the plan of care, options and available services to effectively meet the
Patient and family's needs.
Educate and advocate for patients/families to enhance their ability to make informed decisions regarding medical care, discharge planning, advance directives and end of life care including actively participating in the guardianship process when appropriate.
Required Qualifications:
MSW degree from a CSWE accredited School of Social Work required.
Medical or Hospital social work experience a plus
Experience with diverse cultures and populations preferred.
Licenses and Certifications:
Must have a valid driver's license.
Special Skills, Knowledge Required:
Bilingual preferred.
