Wage Range: $96,782 - $154,835 per year
Posted wage ranges represent the entire range from minimum to maximum. For jobs with more than one level, the posted range reflects the minimum of the lowest level and the maximum of the highest level. Some positions also offer additional premiums based on shift, certifications, or degrees. Job offers are determined based on a candidate's years of relevant experience, level of education and internal equity.
As a key member of EvergreenHealth’s Stroke Center the Nurse Navigator- Stroke serves as a patient-centered stroke resource from admit to discharge to outpatient follow-up. This Nurse Navigator provides patient education and assistance as needed with accessing clinical and supportive care services offered by EvergreenHealth and the community. After discharge this Nurse Navigator will remain point of contact for families and patients. The Nurse Navigator- Stroke functions on the multidisciplinary team as an advocate, educator, and counselor for stroke patients.
Primary Duties:
1. Develops and maintains discharge follow-up plan to evaluate functional outcomes as required by certification bodies.
2. Assists patients in understanding their diagnosis, treatment options, and the resources available.
3. Educates eligible patients about appropriate supportive care resources, research studies and technologies available to this unique patient population.
4. Follows patients throughout the care continuum, including inpatient admissions, and collaborates with inpatient, outpatient, and community care management resources.
5. Performs other duties as assigned.
License, Certification, Education or Experience:
REQUIRED for the position:
● Bachelor’s in Nursing (BSN)
● Licensed as a Registered Nurse in the State of Washington
● 3 years of caring for stroke/cerebrovascular patients
● Current Healthcare Provider BLS certification by date of hire
DESIRED for the position:
● Master’s degree
● SCRN or CNRN certification
