As the Military and Veteran Liaison, you will play a crucial role in providing indirect client care, outreach, and case management services. You will serve as the primary liaison between our facility and outside agencies for discharge planning and continuum of care functions, supporting our facility's treatment program and philosophy.
Key Responsibilities:
  1. Point of Contact: Serve as the point of contact for referring Active Duty Providers and Veterans Affairs (VA) providers, maintaining open lines of communication.
  2. Availability: Be available to accept referrals at any time, ensuring responsiveness beyond regular 8-hour workdays.
  3. Referral Coordination: Coordinate referrals with the Intake team and Clinicians to provide timely feedback to referral sources and schedule admissions.
  4. Discharge Planning: Assist in coordinating discharge planning with Active Duty Providers and Commands, as well as VA providers, to ensure seamless transitions.
  5. Reporting: Ensure that all reports on Active Duty admissions, from admission to discharge, are sent to Active Duty providers promptly through fax or email.
  6. Liaison Role: Serve as a liaison between Active Duty Providers and the Chris Kyle Patriots Hospital team, maintaining up-to-date contact information, phone numbers, and email addresses.
  7. VA Communication: Maintain effective communication with VA referral sources, track patient authorizations for treatment, and facilitate referrals to the next level of VA care if private care is not authorized. Develop and improve communication with VA referral sources.
  8. Business Development: Actively participate in expanding service lines, maintaining/increasing the referral base, and supporting overall business development strategies for the program.
  9. Reserves and National Guard: Work with referrals from the Reserves and National Guard to facilitate the activation of benefits, clarify treatment needs, and coordinate admissions and discharge care. Address the unique mental health care needs of this underserved population.
  10. Patient Satisfaction: Collaborate with the Chris Kyle Patriots Hospital treatment team to enhance patient satisfaction by participating in treatment teams, meeting with patients to address unique needs, and improving the overall treatment experience.

Qualifications
Qualifications: To excel in this role, you should possess:
  • Must be 21 years of age or older
  • Bachelor's Degree in Social Work, Psychology, Counseling, Marketing, or Public Relations or a combination of training and experience
  • Relevant experience in Military and Veteran Liaison roles, with a strong understanding of the military healthcare system.
  • Effective communication and interpersonal skills to engage with a wide range of providers and agencies.
  • Knowledge of VA referral processes and authorization procedures.
  • A commitment to supporting the mental health needs of veterans and active-duty personnel.
