Purpose of Your Position
The essential functions of your job are the following:
Tp build a therapeutic relationship with residents.
Meeting with residents on caseloads for 1:1 at least monthly
Initiate residents’ STP care plan centering interventions on the specific needs of the individual.
To deescalate residents in crisis and provide support following the crisis.
To provide support in clinic with psychiatrist.
Provide nonpharmacological interventions to psychotropic medication care plan.
Collaborate with Interdisciplinary Team for residents' well-being.
Provide ample objective documentation on 1:1 meeting, residents behaviors, progress, and any other information of note.
Help clients identify goals and potential solutions to problems pertaining to mental and behavioral well-being
Seek to improve communication and coping skills, strengthen self-esteem and promote behavior change and optimal Mental Health for all clienteles.
Responsible for planning, implementing and facilitating outcome-based activities required within program.
Provide assistance to help clients develop daily living skills and improve their mental, physical, or emotional health.
Sufficient verbal and written communication skills required to perform tasks required.
Must be knowledgeable and follow Title 22 guidelines.
Delegation of Authority
Program Counselor is delegated the administrative authority, responsibility, and accountability necessary to carry out your assigned duties.

Administrative Functions
Conduct comprehensive assessment of client needs.
Develop Mental Health care plan for clients.
Assist clients and clinicians with weekly psych/clinic visits and document progress.
Assist clients and develop individualized service plans with specific goals and objectives.
Monthly and PRN documentation in patient record.
