What you bring to SBHG:
- Master's degree in child counseling, development or psychology, social services or vocational counseling or related social science field required
- (2) years of experience working in a mental/behavioral health field required.
- Valid California Driver's License required
How you will make a difference:
- Conducts initial and ongoing assessments of the youth and family to identify strengths and needs, to include diagnosing of clients with emotional disturbances.
- Formulates and implements the client's plan of care in collaboration with the Child and Family team.
- Coordinating client's transition to FSP Program with treatment team and transferring program, when applicable.
- Provides direct care services, including individual, group, and family therapy. Also provides case management and collateral services to clients and their families, as deemed necessary and appropriate through the assessment/plan of care process and in collaboration with the Child and Family team.
- Provides crisis intervention to youth and families as needed. Part of an after-hours crisis support on-call rotation.
- Provides and supports others in providing case management and linkage services provided to youth and their families.
- Documents all services provided in a clear and concise manner within the Electronic Health Record within the timelines and framework established by Central Star.
- Completes intake documents, screeners, and assessment forms in various databases. (County EHR, agency EMR, DCR)
- Guides services provided by the Family Specialists, Peer Mentor, and Parent Partner through the CFT meeting process.
- Develops and maintains relationships with child welfare, mental/behavioral health, and other external partners.
- Ensures proper follow up on all referrals made to both internal and external resources.
- Actively participates in the agency's and SBHG's Total Quality Management and Continuous Quality Improvement systems which support compliance with Joint Commission and other accreditation standards as applicable.
- Participates in TQM activities as requested including CQI subcommittees, probes, peer review, audit and/or tracer activities.
- Acts as a liaison between Central Star/families and outside agencies
