Job Description
Under supervision, analyze, abstract, code or index patient's medical record in a municipal hospital, medical center component, skilled nursing facility, ambulatory care facility or health care organization; perform related work
The intent of this position is to provide support to both internal and external customers as it relates to medical record information. Employees in this position will assist in retrieving, scanning and providing information as it relates to requests from attorney offices, insurance companies, medical practices, hospitals, courts or subpoenas as requested.
- Assist the Associate Medical Record Specialist Ill with day to day functions needed to meet assignments within the department
- Gathers electronic or paper medical record information as requested.
- Answers phone call regarding Medical Records or request for information for patients.
- Meets with walk-in patients requesting information on chart requests for various reasons
- Responsible for scanning all paper charts into the system for electronic storage
.• Assists in maintaining the flow of patient information (correspondence) on a daily basis. - Gathers information for the Supervisor as it relates to requests from Attorney offices, insurance companies, medical practices, hospitals, courts or subpoenas as requested
- Investigate and resolve problems presented by patients who call or present requesting information or clarification
- Assist with preparations of surveys and audits as needed
- May supervise and train clerical staff of a medical record department
- May organize, file and retrieve Medical Records
Minimum Qualifications
- High School diploma or its educational equivalent; and
- Completion of a coding, medical record technology or health information technology program approved by the American Health Information Management Association(AHIMA); or
- Possession of a valid certificate as a Certified Coding Specialist (CCS) from the American Health Information Management Association (AHIMA); or
- Two (2) years of satisfactory experience in coding and abstracting Medical Records in a recognized hospital or health care organization.
