POSITION PURPOSE:
The Medical Records Clerk will be responsible for reviewing Medical Records to insure presence of required reports and physicians' signatures, and routes incomplete records to appropriate personnel for completion or prepares reports of incomplete records to notify administration.
KEY RESPONSIBILITIES:
The Medical Records Clerk is responsible for filing, Copying, mailing and/or faxing patient chart information as requested and authorized being sure to properly document all processes. Needs to make sure that all the patient's documents are neatly and correctly filed in the correct chart order.
ADDITIONAL DUTIES:
v Pulls charts, files out guides, and deliver requested charts to designated locations. Replace damage charts as needed, and/or starts a second volume when chart space has been maximized.
v Pull charts for doctor's schedule, add-ons to schedules, patient messages and med refills.
v Assists in purging deceased and inactive charts as scheduled and according to guidelines.
v May be required to maintain a death log.
v File all dictations and operative reports within 24 hours after receiving.
v May compile, verify, and files Medical Records of hospital or clinic patients.
v Check list of discharged patients to insure receipt of all current records.
v Compile daily and periodic statistical data, such as admissions, discharges, deaths, births, and types of treatment rendered.
v Locate charts that are taken out of the Medical Records department.
v May assist other workers with coding of records.
v Document and record all satellite charts for schedule.
v File Medical Records in proper format and checks that all documents have a signature by Doctor(s) and/or Rn(s) before filing in the charts.
v Pick up charts and reports from all medical suites and nurse’s stations.
v Provides back-up assistance as needed by front office staff, and covers for other front office functions as requested.
OTHER DUTIES:
v Responsible and actively involved in the Quality Assessment / Performance Improvement activities
v This person is expected to work beyond scheduled hours if circumstances require it as determined by Administrator
v Accepts and acts per positive constructive criticism to upgrade abilities, as observed by Administrator.
v Uses free time constructively as determined by the Administrator.
v Will perform all other duties as assigned, as determined by Administrator.
v Responsive to patient needs solving patient problems and issues.
v Filing, faxing and copying as requested.
v Prepare correspondence/forms/letters etc. as requested by patients and faculty
v Experience with computerized scheduling systems and multi-line telephones.
v Experience with clinic procedures for pulling and tracking medical charts, x-rays, and other patient related documents.
v Working knowledge of medical terminology
QUALIFICATIONS:
v High School Graduate
v Minimum of one (1) year related experience
v Possess knowledge of modern office equipment, systems and procedures
v Ability to work independently and takes initiative to improve overall office operations
v Strong interpersonal skills
v Computer Literate
v Relies on instructions and pre-established guidelines to perform the functions of the job
v Works with limited supervision
v A certain degree of creativity and latitude is required
v Demonstrates initiative
PHYSICAL REQUIREMENTS:
v Ability to stand for long periods of time
v Ability to lift at least 30 lbs
v Ability to speak distinctly in order to be understood
