JOB SUMMARY
The Medical Coder reports to the Director of Quality & Compliance and is responsible for reviewing patient’s medical records to assign appropriate ICD-10 codes for Hospice, Palliative Care and Memory Partners programs. Provides support for the QAPI program by participating in operational changes and quality committee meetings. Conducts palliative care record audits for content, completeness and regulatory compliance. This individual is vital for ongoing monitoring for accuracy of ICD-10. Performs other duties as assigned.
PRIMARY JOB DUTIES
- Reviews hospice physician documentation for the patient’s primary diagnosis and related co-morbidities upon admission and at each recertification.
- Enters into the electronic medical record (EMR) ICD-10 codes specific to the hospice patient’s primary diagnosis and related co-morbidities based upon the reviewed hospice physician’s documentation.
- Enters updates into the electronic medical record (EMR) ICD-10 codes for primary and/or related diagnoses upon notification from the hospice physician.
- Performs specific hospice prebilling audits and follows up with clinicians for any error corrections to facilitate timely submission of notice of election.
- Accesses and reviews palliative care documentation in contracted hospitals EMR systems.
- Enters the appropriate ICD-10 codes into the LCFL palliative care business units of the EMR for billing purposes.
- Audites a random sampling of palliative care records for correct CPT codes weekly and reports deficiencies to the Chief Clinical Officer and PFS.
- Reviews EMR of memory partners free text diagnosis and enters ICD-10 codes.
- Provides education and feedback to providers of CPT and ICD-10 coding and updates.
- Acts as backup to the Compliance Specialist.
- Organizes and performs work effectively and efficiently.
- Maintains and adjusts schedule to enhance program performance.
- Demonstrates positive interpersonal relations in dealing with all members of the team.
- Maintains and promotes customer satisfaction. Effectively demonstrates the mission, vision, and values of Lower Cape Fear LifeCare on a daily basis.
JOB SPECIFICATIONS
- Education: Two-year associates degree in Medical Coding.
- Licensure / Certifications: Certification as a Coding Specialist (CCS), or equivalent.
- Experience: 1-2 years medical coding experience in a healthcare setting.
- Essential Technical / Motor Skills: Manual dexterity and skill to operate the following: computer keyboard, telephone system, copy machine, fax machine. Verbal, written and computer skills necessary to communicate with internal and external customers.
- Interpersonal Skills: Ability to interact with other staff in a positive and professional manner.
- Essential Physical Requirements: Must be able to sit for extended periods of time and bend/twist/pull/lift/ambulate.
- Essential Mental Abilities: Ability to apply judgment and reasoning to prioritize work, make appropriate decisions, and understand, follow and recall verbal and written instructions.
- Essential Sensory Requirements: Ability to use visual and auditory senses and verbally communicate.
- Exposure to Hazards: Reasonably anticipated occupational exposure related to an office environment.
- Other - Hours of Work: 8-hour flexible day shift Monday through Friday. Flexibility of these hours may be required to meet job demands. 16 hours per week.
