Medical Case Manager

Job Summary: This position is responsible for providing Medical Case management and nursing services needed by Daily Planet patients living with, or at high risk for HIV. The Medical Case Manager will work with our HIV outreach team connecting homeless and other high risk patients to Ryan White services. The goal of this Ryan White position is to recruit and retain patients living with HIV into a treatment program, and to eliminate barriers to adherence with antiviral therapy, thereby decreasing the spread of HIV. The Ryan White responsibilities are expected to consume 60% of this position’s time. The other 40% will be comprised of nursing duties as assigned throughout the DPHS system. These duties may include DPHS staff trainings, and providing staffing support in clinics throughout the agency. The person filling this position will protect the dignity, privacy and confidentiality of patients and their families; and work collaboratively with other Daily Planet providers and staff to promote a team-based approach to patient care.

Job Requirements: RN with Association of Nurses in AIDS Care Certification preferred. Candidates with related experience for two years considered regardless of academic preparation, but must complete a VDH-approved basic case management training program within one year of hire date. All Medical Case Managers must also complete VDH minimum training in HIV CM Standards, HIV 101, cultural competency, and AIDS Drug Assistance Program within their first twelve months. Ability to work independently and as part of a multidisciplinary team. Experience teaching adult learners. Experience working with homeless and patients living with HIV. Skills include excellent oral and written communications including telephone, organizational, and personal computer including word processing and Microsoft office.
